Acute generalized exanthematous pustulosis (AGEP) induced by cefotaxime

Abstract: We report a case of acute generalized exanthematous pustulosis (AGEP) after cefotaxime use confirmed by a positive patch test. A 30-year-old woman received cefotaxime, fosfomycin and ciprofloxacin for sinusitis. Twelve days after drug initiation, she developed an extending pustular erythema associated with fever. Laboratory investigations showed marked leukocytosis. His blood chemistry was normal. The histological examination showed parakeratosis, spongiosis and nonfollicular intra-epidermal pustules consisten… Show more

“…Mucous membrane involvement, detected in 20% of AGEP cases [1], was present in the current case. In 80% of AGEP cases, leukocytosis, mostly caused by a blood neutrophil count above 7,000/mm 3 , is noted; mild eosinophilia is reported in 30% [5]. The laboratory findings of the current case were also comparable.…”

“…AGEP is usually induced by antibiotics (80% of all cases, particularly β-lactam and macrolides), NSAIDs, analgesic, antiarrhythmic (calcium channel blockers, e.g. diltiazem), anticonvulsant and antidepressants [7,8]. Clinically, it is often difficult to distinguish between AGEP and generalized pustular prosiasis or subcorneal pustulosis [7].…”
