Acute Decompensated Heart Failure

Abstract: Heart failure (HF) is a societal burden due to its high prevalence, frequent admissions for acute decompensated heart failure (ADHF), and the economic impact of direct and indirect costs associated with HF and ADHF. Common etiologies of ADHF include medication and diet noncompliance, arrhythmias, deterioration in renal function, poorly controlled hypertension, myocardial infarction, and infections. Appropriate medical management of ADHF in patients is guided by the identification of signs and symptoms of fluid… Show more

“…Some mechanisms can be proposed, such as systemic infection effects on HF haemodynamics and mechanisms related to COVID‐19. Infection is a common cause of acute DHF (ADHF) 5 . Pulmonary infection was one of the most prevalent factors precipitating ADHF and was reported to be independently associated with hospital mortality 6,7 .…”

“…Infection is a common cause of acute DHF (ADHF). 5 Pulmonary infection was one of the most prevalent factors precipitating ADHF and was reported to be independently associated with hospital mortality. 6,7 Although the exact cause is still unclear, activation of inflammatory and immunological pathways may lead to systemic effects and myocardial dysfunction.…”

“…Additional potential usage of SGLT2i may be anticipated for the treatment of acute HF (AHF), which is the major cause of HHF, and for which there are no currently available therapies that improve clinical outcomes [96]. The EMPA Acute Heart Failure, EMPAG-HF and EMPULSE trials are underway to study the effects of SGLT2i on clinical outcomes in the AHF population (ClinicalTrials.gov Identifiers: NCT03554200, NCT04049045 and NCT04157751, respectively), while the EMPA-RESPONSE study has been published recently [97].…”

“…Acute decompensated heart failure (ADHF) refers to the acute attack of symptoms and signs of HF [2]. The common causes of ADHF include drug and dietary disorders, arrhythmia, deterioration of renal function, poor control of hypertension, myocardial infarction, and infection [3]. Risk of readmission within 6 months was as high as 50% for ADHF patients hospitalized due to worsening HF [4], and repeated hospitalization will seriously affect the quality of life of ADHF patients.…”
