Acute coronary syndrome presenting after pseudoephedrine use and regression with beta-blocker therapy

Abstract: Pseudoephedrine, a common ingredient in cold relief drugs, dietary supplements and Chinese herbal tea, has potent sympathomimetic effects, impacting the cardiovascular system. The chemical properties and clinical effects of pseudoephedrine are similar to those of ephedrine, and its main effect is caused by the release of endogenous norepinephrine. “…As the highest dose represents half the LD 50 , we could be reasonably assured that any effects of PSE would be evident, but the dose was not high enough to cause seizure or death in the absence of HBO. It should be noted that high doses of PSE have been shown to cause cardiovascular complications (Akay and Ozdemir ; Fidan et al. ) and that these may affect LS by altering circulation or cerebral perfusion; however, we posit that this is simply an additional mechanism by which PSE may affect divers and does not confound interpretation of the results.…”

“…Case reports suggesting that pseudoephedrine could cause tachycardia, coronary artery spasm and MI, during its use for symptomatic treatment, have been published [4,5,15]. It is interesting to note that these suggested complications were seen in cases without coronary disease.…”
