S Amiodarone Lung toxicity: case reportA 62-year-old man developed lung toxicity following therapy with amiodarone.The man underwent abdominal surgery for biliary leak. Postoperatively, he developed atrial fibrillation with rapid ventricular response. Therefore, he received IV amiodarone 150mg bolus followed by amiodarone drip at 1 mg/hour for 48 hours. One day later, he developed progressive shortness of breath with hypoxemic respiratory failure. He received noninvasive positive pressure ventilation and oxygen. His chest radiograph revealed evidence of new onset bilateral alveolarinterstitial infiltrate and minimal pleural effusion. Seventy-two hours after amiodarone, his chest CT angiogram revealed pulmonary ground-glass opacities and foci of consolidation. Diagnosis of acute amiodarone-induced lung toxicity was made. A differential of bronchoalveolar lavage revealed the presence of 43% of neutrophils and 46% of macrophages, 70% of which were foamy macrophages [outcome not stated].
