Latar belakang. Efektivitas terapi akupunktur untuk tatalaksana adjuvan nyeri kanker pada anak masih kontroversial. Tujuan. Melakukan telaah kritis untuk menilai efektivitas terapi akupunktur untuk tatalaksana adjuvan nyeri kanker pada anak. Metode. Penelusuran artikel dilakukan melalui database Pubmed dan Cochrane dengan menggunakan kata kunci yang sesuai pada bulan Desember 2019. Hasil. Terdapat tiga artikel yang berguna berupa satu penelitian kohort prospektif, satu penelitian uji klinis acak, dan satu meta analisis Pasien berusia remaja cenderung lebih dapat menerima terapi akupunktur sebagai tatalaksana adjuvant nyeri kanker. Pada uji klinis acak dan meta analisis, didapati bahwa pasien yang mendapat terapi akupunktur memiliki skor nyeri yang lebih rendah. Meskipun demikian, penelitian uji klinis acak dan meta analisis tersebut tidak mengikutsertakan pasien anak (usia kurang dari 18 tahun). Kesimpulan. Terapi akupunktur merupakan suatu modalitas tatalaksana adjuvan nyeri kanker yang cukup potensial, tetapi diperlukan penelitian lebih lanjut pada populasi anak guna membuktikan efektivitas terapi tersebut.

Application of Complementary and Alternative Medicine Therapy on Patient with Postoperative Pain After Cardiac Surgery: a Literature Review

Pain is a devastating outcome following cardiac surgery due to routine procedures, including chest tube removal, deep breathing, and coughing exercise. Pharmacological treatment requires complementary interventions in addressing the post-acute pain phase. This study aimed to identify the application of complementary and alternative medicine (CAM) to reduce postoperative pain after cardiac surgery. A literature search was conducted by exploring original studies on the Google Scholar, PubMed NCBI, ProQuest, and ScienceDirect database that published in the last five years (2016-2021), enrolled adult participants who were undergoing cardiac surgery, and evaluated the outcome of CAM therapy on the postoperative pain after the cardiac surgery. Eighteen original studies were analyzed and synthesized. Thirteen studies employed randomized controlled trial, four studies applied quasi-experimental, and one study used one group pre-post-test design. Among these studies, ten studies focused on the manipulative and body-based practices domain (cryotherapy and massage). The application of relaxation exercise, music therapy, and distraction technique from the mind-body intervention domain was discussed in six studies. Only three studies reported the utilization of therapy from the alternative medical system, biologically-based therapy, and energy therapy domain. The majority of the CAM interventions provided significant effects in postoperative pain relief after cardiac surgery. Nurses should be able to assign appropriate CAM therapies for each patient, although all domains of CAM consider as an effective modality for postoperative pain relief after cardiac surgery. Hence, the CAM therapy would be a proper complement for the pharmacological therapies, effective in reducing the pain, and provides a chance for the patient to manage their pain independently.

Acupressure was used to treat adverse effects in children (n = 85) receiving HSCT, with success in reducing nausea and vomiting, fatigue, depression, and anxiety when compared with patients who received only standard of care 22 . Background: Hematopoietic stem cell transplant (HSCT) is associated with significant morbidity and high symptom burden including mucositis pain, nausea, and vomiting.There is little documentation in the literature regarding acupuncture or acupressure for children undergoing HSCT. Objective: The purpose of this study was to determine the safety and acceptance of acupuncture and acupressure in children undergoing HSCT in a large tertiary care children's hospital. Methods: This is a descriptive retrospective study that evaluated 80 admissions to the HSCT unit over a 24-month period. Every child admitted for HSCT was offered acupuncture or acupressure as part of their care.Results: Of 80 patients, 46 were male patients (ages range, 0-32 years; mean, 8 years). Diagnoses include leukemia/lymphoma, sickle cell disease, aplastic anemia, neuroblastoma, and other metabolic, other solid tumor, or other hematologic disorders.Both allogenic and autologous were represented. Sixty-six patients (82.5%) agreed to treatment with acupuncture, acupressure, or both. There were no adverse effects or safety concerns noted. Symptoms addressed by acupuncture or acupressure included pain from mucositis, nausea, constipation, diarrhea, anxiety, insomnia, and general wellness or healing. Conclusion: This study demonstrates that acupuncture and acupressure are well accepted by children and their families admitted for HSCT and are safe even when performed in the first 4 weeks following transplant when the patient is likely to have thrombocytopenia. Implications for Practice: These findings suggest that there may be more therapies for patients undergoing HSCT to help with symptom control.
