ЦЕЛЬ. Изучить информативность ультразвукового исследования (УЗИ) у больных с параколлярным абсцессом, оценить лечебно-диагностическую эффективность мини-инвазивного лечения абсцессов брюшной полости и малого таза при данной патологии. МЕТОДЫ И МАТЕРИАЛЫ. В период 2016-2021 гг. пролечены 85 больных, перенесших мини-инвазивное дренирование параколлярных абсцессов под УЗИ-и рентгенотелевизионным (РТВ) наведением. У 72 (84,7 %) больных диагноз подтвержден данными мультиспиральной компьютерной томографии. У 12 (14,1 %) пациентов выполнено трансвагинальное, у 25 (29,4 %) -трансректальное УЗИ. В 18 (21,1 %) случаях диагноз был установлен на основании магнитно-резонансной томографии малого таза. РЕЗУЛЬТАТЫ. Дивертикулит у 65 (76,4 %) больных привел к развитию параколита, с образованием у 40 (47 %) внутрибрюшных и у 25 (29,4 %) -забрюшинных инфильтратов. Перкутанное дренирование выполнено в 85 случаях. По объему абсцессы разделились: у 19 (22,3 %) -8-10 см; у 42 (49,4 %) человек -5-8 см; в 24 (28,2 %) наблюдениях -до 5 см. Наиболее тяжелыми были пациенты с объемом абсцесса от 8 до 10 см. Время стояния дренажей у пациентов с размерами 8-10 см -16-18 дней, 5-8 см -10-14 дней, до 5 см -не более 10 суток. Радикальное оперативное лечение выполнено у 15 (17,6 %) пациентов. ЗАКЛЮЧЕНИЕ. Параколлярный абсцесс является показанием к его мини-инвазивному дренированию. Миниинвазивное дренирование нами рассматривалось как возможный «мост» к плановой хирургии дивертикулярной болезни толстой кишки (ДБТК). УЗИ обладает высокой чувствительностью и специфичностью при ДБТК. Это позволяет использовать его не только как метод первичного исследования и мини-инвазивного дренирования гнойника, но и как способ объективизации за динамикой состояния пораженного участка толстой кишки.
