Background: Increasing evidence implies that Adipocytokines may result in cardiovascular disease (CVD) events and metabolic changes in the general population and also increase graft failure rate in the renal transplant recipient. Objectives: To compare the serum levels of Adipocytokines and lipid profiles in renal transplant recipients with healthy individuals. Methods: In a case-control study undertaken from the beginning of 2015 to December 2016; 30 renal transplant recipients, with stable conditions, whose renal transplant at least survived well over six months, were randomly selected to be the case group. Besides, 30 healthy individuals who referred to the transplantation clinic as the patients' attendants were considered as the control group. The serum levels of IL-1, IL-6, TNF-α, Adiponectin, Visfatin, Leptin, and the Lipid profiles were measured after 12 hours of fasting and were compared between the two groups. Results: The serum levels of Adipocytokines including IL-1, IL-6, TNF-α, Visfatin, and Leptin were significantly higher in renal transplant recipients than in healthy individuals (P <0.001) and the serum level of adiponectin (P < 0.004) was significantly lower in the renal transplant recipients. Linear regression results indicated that in the renal transplant group, only the Visfatin had a significant negative correlation with age (Unstandardized Coefficient = -0.104, P = 0.002). There were significant positive correlations between Leptin with cholesterol (P = 0.005), triglyceride (P = 0.008), and LDL (P = 0.014) in the healthy individuals, however, these relationships were not found in the renal transplant recipients and only a significant negative correlation was found between Tumor necrosis factor alpha (TNF-α) and cholesterol levels (P = 0.01). Conclusions:The findings of the present study demonstrated that renal transplant recipients suffer from inflammation and accompanying changes in levels of Adipocytokines in comparison with healthy controls.

IntroductIon Visceral adipose tissue is the main source of circulating proinflammatory adipokine, visfatin/nicotinamide phosphoribosyltransferase (visfatin/NAMPT), whose role in the pathogenesis of metabolic syndrome (MS) components such as hypertension and carbohydrate and lipid disturbances is still uncertain, due to commonly used low specific C-terminal immunoassays to determine visfatin/ NAMPT levels. objectIves The aim of the study was to assess the association between the occurrence of MS components and circulating visfatin/NAMPT levels in elderly population. PAtIents And methods The analysis included 2174 elderly participants of the PolSenior study without heart failure, severe chronic kidney disease, cancer, and malnutrition. MS was defined according to the modified International Diabetes Federation criteria. Plasma visfatin/NAMPT concentrations were measured by a highly specific enzyme-linked immunosorbent assay. Additionally, high-sensitivity C-reactive protein (hsCRP), interleukin 6 (IL-6), and insulin levels were assessed, and the homeostasis model assessment for insulin resistance was calculated. results Women were diagnosed with MS more often than men (71.2% vs 56.8%; P <0.001) and had a greater prevalence of all MS components except for type 2 diabetes. Women with MS had higher concentrations of hsCRP and IL-6 than those without MS. Visfatin/NAMPT concentrations were higher in women with MS than in those without MS (1.06 ng/ml [0.65-1.87] vs 0.85 ng/ml [0.54-1.40]; P <0.001), but no differences were observed in men (0.97 ng/ml [0.59-1.61] vs 0.90 ng/ml [0.56-1.60], respectively; P = 0.5). In women, there was a stronger association between the number of components of MS and increased plasma visfatin/NAMPT levels than in men. conclusIons Plasma visfatin/NAMPT levels are increased only in elderly women with MS. Nicotinamide phosphoribosyltransferase Nicotinamide phosphoribosyltransferase (Nampt Nampt ) is a key nicotinamide adenine dinucleotide (NAD) NAD biosynthetic enzyme in mammals, converting nicotinamide nicotinamide into nicotinamide mononucleotide nicotinamide mononucleotide (NMN NMN ), an NAD intermediate. First identified in humans as a cytokine cytokine pre-B-cell colony enhancing factor pre-B cell colony enhancing factor (PBEF PBEF ) and subsequently described as an insulin-mimetic hormone visfatin visfatin , Nampt has recently excited the scientific interest of researchers from diverse fields, including NAD biology, metabolic regulation, and inflammation. As an NAD biosynthetic enzyme, Nampt regulates the activity of NAD-consuming enzymes such as sirtuins sirtuins and influences a variety of metabolic and stress responses. Nampt plays an important role in the regulation of insulin secretion insulin secretion in pancreatic β-cells. Nampt also functions as an immunomodulatory cytokine cytokine and is involved in the regulation of inflammatory responses. This chapter summarizes the various functional aspects of Nampt and discusses its potential roles in diseases, with special focus on type 2 diabetes mellitus (T2DM).
