Active Methamphetamine Use is Associated with Transmitted Drug Resistance to Non-Nucleoside Reverse Transcriptase Inhibitors in Individuals with HIV Infection of Unknown Duration

Abstract: Background:Frequent methamphetamine use among recently HIV infected individuals is associated with transmitted drug resistance (TDR) to non-nucleoside reverse transcriptase inhibitors (NNRTI); however, the reversion time of TDR to drug susceptible HIV may exceed 3 years. We assessed whether recreational substance use is associated with detectable TDR among individuals newly diagnosed with HIV infection of unknown duration.Design:Cross-sectional analysis. Methods:Subjects were enrolled at the University Californ… Show more

“…One plausible explanation is that recent research showing reduced transmission risk among HIV-positive individuals on antiretroviral therapy (Cohen et al, 2011;Donnell et al, 2010;Grant et al, 2010) could affect risk perceptions and resulting behaviors. Although one of many possible explanations, this could be particularly important to examine in settings where substance use is high (see Kalichman et al, 2011), because substance use has been shown to negatively affect HIV treatment adherence and viral load counts (Arnsten et al, 2002;Baum et al, 2009Baum et al, , 2010Fairbairn et al, 2011;Hahn and Samet, 2010;King et al, 2009;Shacham et al, 2011;Toussi et al, 2009) as well as resistance to antiretroviral medications (Cachay et al, 2007;Colfax et al, 2007).…”

“…Among HIV-positive individuals, heavy alcohol use has been found to be associated with decreased cluster of differentiation 4 (CD4) cell counts or nonsuppression of HIV viral load (Baum et al, 2010;Hahn and Samet, 2010;Samet et al, 2007;Shacham et al, 2011), increased neurodegeneration or memory impairment (Fama et al, 2009(Fama et al, , 2011Persidsky et al, 2011), decreased health-related quality of life (Uphold et al, 2007), and increased mortality from various causes (Bonacini, 2011;Braithwaite et al, 2007;Katcher et al, 2010). Likewise, crystal methamphetamine use among HIV-positive individuals has been found to be associated with decreased CD4 cell counts or nonsuppression of HIV viral load (Fairbairn et al, 2011;King et al, 2009;Toussi et al, 2009), neurode-generation or memory impairment (Scott et al, 2007), and resistance to antiretroviral medications (Cachay et al, 2007;Colfax et al, 2007). Cocaine or crack-cocaine use among HIV-positive individuals has been found to be associated with decreased CD4 cell counts or nonsuppression of HIV viral load (Arnsten et al, 2002;Baum et al, 2009;Rafi e et al, 2011), increases in hypertensive renal changes (Fine et al, 2007), and coronary stenosis (Lai et al, 2008).…”

“…For example, Cachay et al reported that in newly diagnosed, ART naïve individuals, those who reported using METH within the 30 days were more likely to have transmission of drug resistant HIV-1 [37]. In another case study, Markowitz et al reported an association between METH use with infection of a multidrug resistant, dual-tropic HIV-1 that resulted in rapid progression to AIDS[38].…”
