Action tremor in Parkinson's disease: frequency and relationship to motor and non‐motor signs

Abstract: Action tremor is a relatively frequent motor sign in patients with Parkinson's disease staging 1-2 on the Hoehn-Yahr scale. Action tremor correlates with rest tremor and rigidity and may be associated with a lower burden of non-motor symptoms. These findings suggest a contribution of non-dopaminergic mechanisms to action tremor pathophysiology.

“…We used the Mann–Whitney U ‐test to compare the clinical and demographic characteristics (age, age at disease onset, disease duration, dosage and duration of treatment, presence of dyskinesias, motor fluctuations, H&Y scores, MDS‐UPDRS scores and Non‐Motor Symptoms assessment scale for PD scores) of patients with and without RET off and on treatment. In accordance with previous studies , for the statistical analysis we used combined MDS‐UPDRS subscores to summarize the single item scores. The ‘bradykinesia subscore’ included four limbs and the global bradykinesia item scores, the ‘rigidity subscore’ included four limbs and neck item scores, and the ‘axial symptoms subscore’ included speech, facial expression, arising from chair, gait, posture and postural stability item scores.…”

“…Resting tremor is a well‐known feature of PS that also occurs in some ET‐plus cases . Action tremor, which is characteristic of ET, is also a feature of PD . Because of the overlapping tremor manifestations, clinical distinction between ET and PS can be difficult in some cases .…”

“…Resting tremor, which is characteristic of PD, may also occur during the late stages in some ET patients . Action tremor, which is the diagnostic feature of ET, is also present in some PD patients . Additionally, the clinical features of ET and PD each evolve over time .…”

“…Rest tremor was noted at least during one evaluation among 47 pathologically verified Parkinsonian patients [5]. Action tremor was reported in 46–93% of PD patients depending on the selected populations [6, 7]. …”
