Actinomyces-induced inflammatory myofibroblastic tumor of the colon: A rare cause of an abdominal mass

Abstract: HighlightsAbdominal inflammatory myofibroblastic tumor (IMFT) macroscopically appear as mass lesions that grow slowly and sometimes cause destruction of adjacent structures. The exact diagnosis can only be made with histopathological examination of permanent tissue. Resection with negative surgical borders is the most suitable surgical technique. The same principles also apply to IMFT developing on the basis of actinomycosis because it is quite difficult to predict at the preoperative period that the lesion de… Show more

“…There are different theories regarding the etiology of IMTs including trauma, surgery, hemangioma hemorrhage or rupture, bacterial or viral infection, immune disorders and tumor processes[ 7 - 10 ]. Viruses, including HIV, human herpesvirus-8, and Epstein-Barr virus, are presumed to influence the development of IMT[ 11 - 14 ], and there is even a case report of secondary actinomycosis[ 15 ]. However, the actual pathogenesis remains unclear[ 16 ].…”

“…Based on the major component in microscopy, IMT is divided into three histopathological subtypes including xanthogranuloma type, plasma cell granuloma type and sclerosing pseudotumor[ 15 ]. Histologically, IMTs are characterized by spindle cells proliferating in a myxoid-to-collagen matrix with a prominent inflammatory infiltrate mainly composed of plasma cells and lymphocytes, and it is occasionally admixed with eosinophils and neutrophils.…”

“…Actinomyces are usually extremely susceptible to beta-lactams. As for the antibiotic treatment of actinomycosis, penicillin G followed by oral penicillin or amoxicillin for up to 12 months are considered the drugs of choice [ 29 ], and other antibiotics such as erythromycin, clindamycin, or tetracycline can be given, especially in cases with penicillin allergy [ 30 ]. Drug resistance is not a problem in actinomycosis.…”

“…It was difficult to distinguish IPT with or without actinomycosis after the first biopsy because actinomycosis can exhibit local tissue invasion; IPT due to actinomycosis has also been reported ( 11 , 12 ). Second and third tumor biopsies were performed with an 18-G needle because we worried that the tumor samples obtained by the 21G needle had been insufficient to detect infection or malignancy.…”
