Lip squamous cell carcinoma (LSCC) may develop from a potentially malignant disorder known as Actinic cheilitis (AC). The p21-activated kinases (PAKs) are possible targets for cancer therapeutics. The LSSC and AC samples were analyzed by immunohistochemistry in order to detects endogenous levels PAKs group I (PAK1 and PAK2) and group II (PAK4, PAK5 and PAK6) only when they were in phosphorylated form (active). Keratinocytes of the basals stratum showed intense staining for phosphoPAKs 1/2, while the most superficial cells of the stratum spinosum and the surface layer showed moderate immunostaining for PAKs 4/5/6 phosphorylated in the epithelium adjacent to injured area. AC did not immunoexpress actives PAKs 1/2 and PAKs 4/5/6. LSSC did not immunoexpress phosphoPAKs 1/2, but some degenerating cells and cells in the necrotic area showed intense staining. Our results suggest that PAKs 1/2 not participate in the regulation of AC and LSCC pathogenesis, while PAKs 4/5/6 are involved in the regulation of cell death in LSCC.

FUNDAMENTOS: O criopeeling utiliza a crioterapia difusa não somente nas lesões de ceratose actínica, mas em toda a pele fotodanificada. OBJETIVOS: Comparar dois métodos de criopeeling (nitrogênio líquido e sistema portátil de éter dimetílico, propano e isobutano) quanto à eficiência no tratamento de ceratoses actínicas, tolerabilidade do paciente, preferência do paciente e do pesquisador e resultado estético. MÉTODOS: Dezesseis pacientes (n=16) com múltiplas ceratoses actínicas nos antebraços foram submetidos ao criopeeling com nitrogênio líquido em um dos antebraços e com o sistema portátil no outro, randomicamente. RESULTADOS: No tratamento das ceratoses actínicas, o nitrogênio líquido obteve 74% de eficiência e o sistema portátil, 62% (p=0,019). A média da escala visual analógica (0-10) foi 5,7±1,61 com o nitrogênio líquido e 4,3±1,44 com o sistema portátil (p=0,003). Não houve diferença estatística entre os métodos quanto à preferência do paciente e do pesquisador. Na análise das fotos, observou-se melhora do aspecto da pele nos dois tratamentos (p<0,001). Com o nitrogênio líquido, em 62,5% das vezes houve algum grau de melhora; com o sistema portátil, em 52% (p>0,05). CONCLUSÕES: A técnica de criopeeling pode ser uma opção no tratamento de fotodano. O sistema portátil pode ser uma alternativa interessante na prática clínica, com boa tolerância e resultados aceitáveis no tratamento de ceratoses actínicas

Skin cancer detection using laser imaging

Endeavors to develop a non-invasive and reliable device to detect different types of skin cancer in its early stages have been undertaken over the past few years. Although some advances have been made towards such developments, there remains great scope for the development of a non-expensive, compact, reliable, safe, easy to use, fast, sensitive, and non-invasive device. This project will apply the innovative laser feedback interferometry (LFI) technique to skin cancer detection via laser imaging using near-infrared vertical cavity surface emitting lasers (VCSELs). In this project, the science behind lighttissue interactions has been explored and skin imaging modalities have been elaborately studied. As a result, LFI has been exploited in various configurations to image different modalities such as Doppler flowmetry and confocal reflectance. In addition to single modality imaging, dual-modality imaging has also been proposed and conducted experimentally, to image Doppler flowmetry and confocal reflectance at the same time, using an integrated LFI system. It is shown that dual-modality imaging can yield images with higher contrast which provide more information about morphological and functional characteristics of a target under test.In addition to two-dimensional LFI imaging, three-dimensional imaging systems have also been studied and employed in the laboratory. A tomographic image identifies tissue volume with altered optical properties due to cancerous or other malignant tissue processes. We applied the three-dimensional imaging system to different types of keratinocyte skin cancer phantoms and results show high level of sensitivity in providing tomographic images. Using high numerical aperture lenses, making images at resolutions down to micrometer scales is possible, which is promising for microscopy of biological structures.Furthermore, parallel LFI imaging using an array of 24 VCSELs was also studied theoretically and experimentally as a part of this work. A VCSEL array was used in a parallel read-out LFI imaging system to develop a fast and high resolution device, which eliminates the need to scan mechanically in one dimension.Investigating the complex interactions of the laser beam and tissue requires a powerful and robust simulation technique. Monte Carlo was used as a simulation technique to model the light-tissue interactions occurring in the optical systems in this work. Firstly, we tested and validated the Monte Carlo main engine which was developed in the group, by studying morphological aspects of the simulated and experimental signals in a laser Doppler velocimetry system, and deep insight into the nature of the LFI Doppler spectrum and further conclusions were drawn.
