Actinic cheilitis: Evolution to squamous cell carcinoma after carbon dioxide laser vaporization. A study of 43 cases

Abstract: CO(2) laser vaporization with an adequate postoperative follow-up is an effective treatment for chronic AC. Nevertheless, some patients (4,6%) went on to develop lip SCC. The follow-up of these patients is mandatory.

“…Good cure rates are achieved with scalpel and CO 2 laser vermilionectomy. 52 Further studies are warranted to compare the efficacies and adverse event profiles of the CO 2 laser and PDT for AC, especially in cases with mild to moderate dysplasia. Scalpel vermilionectomy is first line for diffuse AC with severe dysplasia.…”

“…Patients with evidence of photodamage or a history of AKs should be regularly monitored for new lesions, with an increase in monitoring frequency if any of the following apply: History of NMSC History of nonresponsive AKs Ongoing systemic immunosuppression NA Strong 6. ACs may be treated with any of the following modalities: Cryosurgery Field-directed topical agents, d including 5-fluorouracil and imiquimod PDT c Complete or partial vermilionectomy Laser resurfacing Electrodesiccation and curettage All low 80,85,88 Low 36,86-89 Low 83 Low 91,92 Low 93 Strong Weak Weak Weak Strong Lesions that do not respond to these measures should be biopsied to analyze for evidence of NMSC and treated accordingly. NA Strong 7. Organ transplant recipients and others with longterm systemic immunosuppression and clinical evidence of AKs may receive field-directed therapies to prevent the emergence of AKs and NMSC in areas of photodamage. “…10 Nonetheless, secondary complications, including pain, edema, delayed re-epithelialization, and scarring, have been reported, probably owing to its deep ablative thermal effects. CO 2 laser ablative treatments for AC have been studied extensively and have demonstrated remarkable curative and cosmetic results.…”

“…CO 2 laser is also associated with low recurrence; three of 43 patients were found to have recurrences, of whom two developed SCC. 10 Nonetheless, secondary complications, including pain, edema, delayed re-epithelialization, and scarring, have been reported, probably owing to its deep ablative thermal effects. The 2,940-nm Er:YAG was introduced as an alternative solution.…”
