Background Actinic keratosis (AK) and cheilitis (AC) are lesions that develop on photodamaged skin and may progress to form invasive squamous cell carcinomas (SCCs). Objective To provide guidance to Canadian health care practitioners regarding management of AKs and ACs. Methods Literature searches and development of graded recommendations were carried out as discussed in the accompanying introduction (chapter 1 of the NMSC guidelines). Results Treatment of AKs allows for secondary prevention of skin cancer in sun-damaged skin. Because it is impossible to predict whether a given AK will regress, persist, or progress, AKs should ideally be treated. This chapter discusses options for the management of AKs and ACs. Conclusions Treatment options include surgical removal, topical treatment, and photodynamic therapy. Combined modalities may be used in case of inadequate response. AKs are particularly common following the longterm immunosuppression in organ transplant patients, who should be monitored frequently to identify emerging lesions that require surgery.

Aktinik Şelitis: Tanı, Tedavi, Korunma Ö ÖZ ZE ET T Aktinik şelitis, aynı zamanda eksfolyatif şelitis, solar şeliozis ve dudakların aktinik keratozu olarak da bilinen, skuamoz hücreli karsinom geliştirme potansiyeli olan, dudağın premalign bir lezyonudur. Klinik olarak, dudakta kuruma, pullanma, ülsere ve atrofik alanları içeren, vermilyon deri hattında silikleşme ile karakterize bir lezyondur. Güneşin aktinik etkilerine maruz kalan açık tenli bireylerde daha sık rastlanmaktadır. AŞ tedavisi; periyodik muayeneler alana ve lezyona yönelik tedavi, güneşten korunma ve hasta eğitimi başlıkları altında incelenen cerrahi ve medikal tedavileri içermek-tedir. Bu çalışmada, alt dudağında iyileşmeyen yara şikâyetiyle kliniğimize başvuran, 68 yaşında, beyaz tenli, bir kadın olgunun tanı, tedavi ve postoperatif izlem süreci güncel literatür ışığında tartışılmıştır.A An na ah ht ta ar r K Ke el li im me el le er r: : Aktinik şelitis; cerrahi, oral A AB BS ST TR RA AC CT T Actinic cheilitis is a premalignant lesion that has a potential to transform into squamous cell carcinoma and is also named as solar cheliosis or actinic cheilitis of the lips. Clinically dried, scaly lips; atrophic, ulcerative areas on vermillion and loss of vermillion skin border are typical characteristics. It is common among caucasians who are exposed to actinic effects of sun. Treatment includes surgical and medical modalities that can be summarised as, regular reviews, treatment targeting the lesion and the surrounding tissues, protection from UV rays and patient education. This case report aims to discuss diagnosis, treatment and post-operative review process of a 68 years old caucasian lady who complained of an unhealing ulcer on her lower lip in the light of current literature.

Actinic cheilitis can progress to squamous cell carcinoma in 20% of cases. The purpose of this study was to identify the clinical characteristics of 75 patients with actinic cheilitis and to summarise the current literature concerning actinic cheilitis. A total of 75 patients were evaluated over a 3-year period. The medical history, physical examination results and images were collected from each patient. Demographic data were collected and analysed. To review the literature, the relevant data were collected from scientific journals in the last 30 years. The mean age of the patients was 56 years old; 42 patients (56%) were female, and 66 patients (88%) were white. Nineteen (25.3%) patients reported at least one symptom, including pain, burning and itching. Sixty-five (86.7%) patients presented actinic cheilitis only in the lower lip. All of the patients reported sun exposure, and 44 (58.6%) patients were exposed for more than 10 years. The main clinical aspects investigated and analysed included dryness (100%), flaking (72%) and white lesions (57.3%). Lip assessment is extremely important in medical and dental care. From the 15 lesions assessed in the presence of actinic cheilitis, the most common clinical characteristics observed were dryness, flaking and white lesions. It is imperative to know the clinical aspects that may be encountered in actinic cheilitis in order to achieve early diagnosis, thus avoiding lip squamous cell carcinoma transformation.
