Actinic cheilitis: a treatment review

Abstract: All other factors being equal, the presence of actinic cheilitis, a pre-invasive malignant lesion of the lips, doubles the risk of squamous cell carcinoma developing in this anatomic area. Various forms of local ablation, immunomodulation and surgical extirpation have been proposed as therapeutic interventions. This paper critically evaluates the available medical literature to highlight the evidence-based strength of each recommended therapy for actinic cheilitis. Vermilionectomy remains the gold standard for … Show more

“…80 ACs are found almost exclusively on the lower lip and are less common in women, apparently because of the ultraviolet protection conferred by lipstick. 81 As with other AKs, palpation may be helpful in the diagnosis of ACs. 82 Histologic analysis of the vermilion surrounding ACs commonly identifies foci of SCC.…”

“…81 Less invasive field-directed approaches include topical treatment with 5% 5FU, 85 ALA-PDT, 36,86,87 and MAL-PDT. 88,89 Imiquimod has also been explored for AC; despite Canadian labelling that restricts use of 3.75% and 5% imiquimod on the lips, this approach is common in clinical practice.…”

“…These patients require aggressive treatment, according to the clinical evolution and histologic features [2] [18] [19]. Histopathological results will determine the type of approach, and treatment modalities are numerous, each with its own advantages and disadvantages [19]. One of the most common treatments is vermilionectomy, and other modalities include CO 2 or erbium lasers, electrodessication, topical application of 5-fluorouracil tretinoin, topical imiquimod, trichloroacetic acid chemical peel, photodynamic therapy using 5-aminolevulinic acid (5-ALA) and thulium laser [10] [19]- [22].…”
