Experience with tetracosactrin-depot therapy in 10 patients with dermatological disorders is described. The clinical response was considered good in 7 patients, poor in the remaining 3. Plasma cortisol levels remained elevated 24 to 48 hr. following injection but there were considerable differences between patients in both the magnitude and duration of response.Melanoderma and other side efFects were troublesome when the total weekly dose exceeded 1 mg. Smaller dosages gave satisfactory response without significant accompanying side effects.

Reactions against ACTH were rather commonly noted during the first decade of therapy with pituitary extracts of animal origin. Most of the allergic reactions then reported were probably the result of sensitization against the contaminating foreign proteins in the preparations rather than against the hormone character per se (22). Since the sixties, when highly purified corticotropine came into use, such reactions are less common. The immunological responses may range from fatal allergic shock to the mere occurrence of demonstrable IgE, IgG and IgM antibodies (i, 8, 9, 10, 16, 17, 23, 26, 29, 32).The naturally occurring ACTH molecule is composed of 39 amino acids. The sequence of the amino acids, 25-33, express the species-specific differences (20). The first synthetic ACTH-preparation produced (tetracosactid) is composed of 24 amino acids linked in the same sequence as the first 24 of the naturally occurring ACTH molecule. This preparation was therefore thought to involve hardly any risk of sensitization when administered to human subjects. However, reactions after treatment with synthetic ACTH-preparations have occasionally appeared hoth clinically and experimentally. 0. Antibodies to synthetic ACTH in asthmatic and rheumatic patients. Acta path. microbiol. scand. Sect. C, 84: [353][354][355][356][357][358][359][360][361][362][363][364] 1976.The prevalence of antibodies against synthetic ACTH preparations and porcine gamma globulin was investigated in 29 asthmatic patients, in 28 rheumatic patients, and in 63 normal subjects of both sexes. Agglutinating antibodies were examined by a passive haemagglutination test and IgE antibodies by a radio-immuno assay. The incidence of agglutinating reactions against SynacthCnB and porcine gamma globulin was significantly higher in the asthmatic and rheumatic groups than in the group of normal subjects. Titres were generally low, attaining supposedly pathological levels ( 2 1/200) in about 10 per cent of the asthmatic patients and in 20-40 per cent of the rheumatic patients. In asthmatic patients, the incidence of ACTH specific IgE was higher than that in the normal group. This difference was not statistically significant in patients on routine treatment but it became satistically significant after ACTH booster treatment. In rheumatic patients the incidence of IgE antibodies against ACTH was significantly lower than that in the normal group. The incidence of agglutinating antibodies against vasopressin in asthmatic and rheumatic patients was also increased as compared with that in normal subjects and the titres were positively correlated to those against ACTH.
