ACR Appropriateness Criteria ® Suspected Osteomyelitis, Septic Arthritis, or Soft Tissue Infection (Excluding Spine and Diabetic Foot)

Abstract: Infection of the musculoskeletal system is a common clinical problem. Differentiating soft tissue from osseous infection often determines the appropriate clinical therapeutic course. Radiographs are the recommend initial imaging examination, and although often not diagnostic in acute osteomyelitis, can provide anatomic evaluation and alternative diagnoses influencing subsequent imaging selection and interpretation. MRI with contrast is the examination of choice for the evaluation of suspected osteomyelitis, an… Show more

“…Others tend to be more localised: pain at the site of infection, swelling and erythema [1]. Recognition of these features should prompt clinicians to obtain blood cultures, a full set of blood tests, inflammatory markers, plain x-ray films and a magnetic resonance imaging (MRI) scan [1,8]. Notably, the symptoms of osteomyelitis can either present themselves acutely, or chronically.…”

“…As a limitation, rst, 67 Ga SPECT/CT is associated with exposure to radiation [24]. We minimized radiation exposure: the radiation dose for whole-body gallium scintigraphy is 14.8 mSv and the computed tomography dose index (CTDI) in our imaging study was 1.3 mGy.…”

“…MRI helps identify foreign bodies by the presence of air or metal susceptibility artifact or adjacent edema or fibrosis. MRI may also be used to exclude associated osteomyelitis (see the ACR Appropriateness Criteria Ò topic on "Suspected Osteomyelitis, Septic Arthritis, or Soft Tissue Infection (Excluding Spine and Diabetic Foot)" [81]) or abscess [77].…”
