ACR Appropriateness Criteria® Acute Mental Status Change, Delirium, and New Onset Psychosis

Abstract: Acute changes in mental status represent a broad collection of symptoms used to describe disorders in mentation and level of arousal, including the more narrowly defined diagnoses of delirium and psychosis. A wide range of precipitating factors may be responsible for symptom onset including infection, intoxication, and metabolic disorders. Neurologic causes that may be detected on neuroimaging include stroke, traumatic brain injury, nonconvulsive seizure, central nervous system infection, tumors, hydrocephalus… Show more

“…Therefore, we suggest that when the clinical indication for brain imaging of patients with COVID-19 is altered mental status, the radiologist should consider reviewing the electronic health record in more detail or discuss the risks and benefits with the requesting physician in order to ensure that the benefits of such examination outweigh the risks in the setting of a viral outbreak. 10 The reported prevalence of stroke in the adult population in the United States is 3%. 11 Indeed, hospitalization for acute infection is associated with a transient increase in the risk of vascular events including stroke, with an odds ratio of 8.0 for 14 days preceding the stroke.…”

“…To further explain the findings on knowledge, simulation in VR was consistent, that is all students experienced all the simulated symptoms in one session. Visiting patients under the supervision, on the other hand, has some limitations such as patients might have different mental status in each visit, leading to presentation of different symptomatology in each session ( 32 ). Moreover, patients might not be willing to or feel safe enough to reveal their inner experiences in a crowded setting, resulting in non-cooperativeness.…”

“…Hence, we propose that if there is a clinical warning for head radiological-imaging of COVID-19 patients with a changed mental status, the radiologist must contemplate studying the health information system (HIS) electronic record in more detail or converse the pros and cons with the main primary clinicians to safeguard all concerned, and hence the paybacks of such investigation offset the dangers and risks in the setting of a viral outbreak. 20 …”
