Acquired Reactive Perforating Collagenosis

Abstract: Acquired reactive perforating collagenosis (ARPC) is commonly recognized as an unusual skin reaction to superficial trauma that is observed in patients with a certain genetic predisposition or underlying diseases, such as diabetes mellitus or renal diseases. We present the unusual case of a 55-year-old female diabetic patient with numerous characteristic dome-shaped nodules, which consisted of central umbilication containing firm keratotic plugs.Key Words: Acquired reactive perforating collagenosis, Diabetes m… Show more

“…This case highlights the importance of good history‐taking and clinical examination to diagnose underlying pathologies. The improvement in our patient by treatment of the lymphoma supports the hypothesis of ARPC as a condition secondary to the lymphomatic disease, due to the trauma of scratching in response to the pruritus …”

“…Pruritus and scratching are the common factor between all these pathologies, and control of pruritus helps clear the lesions . It is unclear if ARPC is primary in these cases, but most authors consider it a reaction to a traumatic stimulation, such as scratching, which induces transepidermal elimination of the degenerated collagen fibres . The disease classically erupts as multiple round plaques and nodules with central hyperkeratotic plugs, distributed on the extremities and back, and sometimes the face and neck .…”

“…Perforating disorders, including KD, reactive perforating collagenosis, elastosis perforans serpiginosa and perforating folliculitis, are a group of uncommon diseases, which are histopathologically characterised by transepidermal elimination 5 6. Although rare, its incidence is significantly increased in patients with chronic renal failure 7–9.…”
