Введение. Проведено комплексное обследование 204 новорожденных, поступивших на лечение и выхаживание в ГУ «РНПЦ "Мать и дитя"».Цель. Провести оценку показателей гемостазиограммы в первые сутки после рождения и клинических проявлений нарушения гемостаза на фоне проводимой антигеморрагической профилактики витаминами группы К у недоношенных новорожденных с очень низкой и экстремально низкой массой тела и врожденной пневмонией.Материалы и методы. Контрольную группу составили 63 здоровых доношенных новорожденных. В группу сравнения включили 46 условно здоровых недоношенных детей. В первую исследуемую группу вошли 48 новорожденных с очень низкой и экстремально низкой массой тела, врожденной пневмонией, которые получили менадион в первые сутки жизни. Во второй исследуемой группе наблюдались 47 новорожденных с очень низкой и экстремально низкой массой тела, врожденной пневмонией, которые получили фитоменадион в первые сутки жизни.Результаты. По данным коагулограммы статистически значимо более низкие значения активированного частичного тромбопластинового времени (р=0,035), коэффициента R (р=0,034), протромбинового времени (р<0,001), международного нормализованного отношения (р<0,001) на фоне высокого протромбинового индекса (р=0,004) у недоношенных новорожденных, получивших фитоменадион, свидетельствуют о более стабильном функционировании системы гемостаза и менее выраженной гипокоагуляции в первой и второй фазах свертывания крови в первые сутки после рождения по сравнению с младенцами, получившими менадион.Выводы. Для профилактики геморрагических расстройств у недоношенных новорожденных с очень низкой и экстремально низкой массой тела и врожденной пневмонией при использовании фитоменадиона частота регистрируемых внутрижелудочковых кровоизлиянийIII–IV степени и риск их развития были статистически значимо ниже, чем у младенцев, получивших менадион. Introduction. A comprehensive examination of 204 newborns who were admitted for treatment and nursing in the state medical center “Mother and child” was conducted.Purpose. To evaluate the hemostasiogram indicators in the first day after birth and clinical manifestations of hemostatic disorders on the background of anti-hemorrhagic prevention with the group K vitamins in premature newborns with very low and extremely low body weight and congenital pneumonia.Materials and methods. The control group consisted of 63 healthy full-term newborns. The comparison group included 46 conditionally healthy premature babies. The first study group included 48 newborns with very low and extremely low body weight, congenital pneumonia, who received menadion in the first day of life. In the second study group, 47 newborns with very low and extremely low body weight, congenital pneumonia received phytomenadion in the first day of life. Results. According to the coagulogram, statistically significantly lower values of the activated partial thromboplastin time (p=0.035), coefficient R (p=0.034), prothrombin time (p<0.001), international normalized ratio (p<0.001) on the background of a high prothrombin index (p=0.004) in premature newborns who received phytomenadion indicate a more stable functioning of the hemostatic system and less pronounced hypocoagulation in the first and second phases of blood clotting in the first day after birth if compared to infants who received menadion.Conclusions. For prevention of hemorrhagic disorders in premature infants with very low and extremely low body weight and congenital pneumonia, when using phytomenadion, the frequency of registered intraventricular hemorrhages of the III-IV degrees and the risk of their development were statistically significantly lower than in infants who received menadion.
