Acquired bilateral melanosis of the neck in perimenopausal women

Abstract: These cases seem to represent a continuum of Riehl melanosis. However, the principal distribution of the pigmentation is a distinguishing feature. Any consistent predisposing factors were not established, but there may be a role for subclinical injury or inflammation as possible causative factors for development of the pigmentation.

“…The vacuolar change of basal layer without spongiosis accompanying dermal melanin deposition, implies the possibility of similar underlying pathomechanism as seen in pigmented contact dermatitis (PCD), also known as Riehl’s melanosis (14). PCD is a rare variant of non-eczematous contact dermatitis, which is believed to be caused by repetitive contacts with very small amounts of allergens, such as washing powder, fragrance or cosmetics (1516).…”

“…In a study of 14 Korean women with acquired bilateral melanosis of the neck consistent with Riehl's melanosis, Park et al . have noted that although some cases showed positive photopatch tests, they were of doubtful relevance . Various treatments, individually as well as in combinations, have been attempted with variable success in Riehl's melanosis …”

“…Natural killer group 2D positive (NKG2D+) cells such as NK, NKT, and CD8+ T cells and NKG2D activating ligands from the MHC I-related chain A family also have a key role in pathogenesis 2 . CD4+/ CD25+ T-cells are also involved 3,4 . Most therapies of AA target the autoimmune reaction, but a new therapy modality is being needed because of limitations and complications of previous therapies 5 .…”

“…On the other hand, follicular keratotic plugs and perifollicular halo were observed only in one and 3 of 9 patients, respectively, at a lower rate than in the face. The major histopathologic features of Riehl's melanosis are vacuolar degeneration of the basal layer and pigment incontinence of the dermis 4 . These pathological features correspond with grey dots/granules on the dermoscopic images that represent melanophages in the dermis 5 .…”
