Acoustic Radiation Force Impulse Imaging for Differentiation of Thyroid Nodules

Abstract: BackgroundAcoustic Radiation Force Impulse (ARFI)-Imaging is an ultrasound-based elastography method enabling quantitative measurement of tissue stiffness. The aim of the present study was to evaluate sensitivity and specificity of ARFI-imaging for differentiation of thyroid nodules and to compare it to the well evaluated qualitative real-time elastography (RTE).MethodsARFI-imaging involves the mechanical excitation of tissue using acoustic pulses to generate localized displacements resulting in shear-wave pro… Show more

“…However, the first three features are known to be very unspecific [21,22], and the other two are less sensitive. In our opinion, the presence of a lesion with the appearance described above should arouse a suspicion of MTC and should be considered as an argument for further diagnostic steps, such as calcitonin measurement.…”

“…A large retrospective study performed by Ahn et al [20] reported that mean size of benign nodules was 17.9 mm, malignant -12.1 mm. In a prospective study performed by Bojunga et al [21], TCs were slightly bigger than other lesions but the difference was statistically insignificant. According to our results, taking into account the size as the most important feature, and US features of malignancy only in a case of lesions presenting equal size, would lead to the omission of 60% of cancers in MNG if one puncture is performed and 40% in case of two punctures per procedure.…”

“…The results are obtained in m/s [4]. Bojunga et al proposed a median velocity in ARFI for healthy thyroid gland (1.76 m/s), benign lesions (1.90 m/s) and malignant tumours (2.69 m/s) [16]. This method also can be used in standard ultrasound devices [4].…”

“…To assess thyroid tissue stiffness, the four-point modified Ueno scale (Table 1) and three-point Rago scale have been proposed, ranging from 0 to 240 kPa and linked with colour-coded scale [16].…”
