ACL injuries and stem cell therapy

Abstract: Tears of the anterior cruciate ligament (ACL) are very frequent injuries, particularly in young and active people. Arthroscopic reconstruction using tendon auto- or allograft represents the gold-standard for the management of ACL tears. Interestingly, the ACL has the potential to heal upon intensive non-surgical rehabilitation procedures. Several biological factors influence this healing process as local intraligamentous cytokines and mainly cell repair mechanisms controlled by stem cells or progenitor cells. … Show more

“…Stem cells, due to their strong self-renewal ability and tissue regeneration capacity, have been studied for the treatment of tissue defect and dysfunction in vitro (5,6). Tendon stem cells have been identified recently and considered an optimal cell source for tendon repair.…”

“…In more recent times, mesenchymal stem cells from different sources have been proposed to augment ACL reconstruction or repair. In this case, flourishing pre-clinical literature suggests that stem cell administration could stimulate tissue maturation, improve histological appearance, and favor bone-to-tendon integration[ 15 , 16 ]. Therefore, the implementation of experimental techniques in the clinical practice seems reasonable.…”

“…Furthermore, ACL injury is often associated with an increased risk of early osteoarthritis of the knee [5, 6]. Several methods of ACL reconstruction using autografts or allografts of the hamstrings or patellar tendon have been under investigation in the last decades for the management of ACL tears [1, 7]. However, the long-term outcomes of current reconstruction methods remain unsatisfactory with high failure rates, thus revisions are necessary [8].…”

“…The anterior cruciate ligament (ACL) plays an important role in stabilization of the knee joints and is one of the most commonly injured ligaments, particularly in young athletes [1]. In response to injury, the ACL has limited regenerative potential due to its limited blood supply and unique intra-articular environment [2-4].…”
