Background There is increasing literature on the usefulness of patient-reported outcome measures (PROMs), but far fewer studies to determine their use by orthopedic surgeons and the barriers they face in applying PROMs in their daily clinical activity. Methods Cross-sectional study using a questionnaire that was distributed in both soft and hard copy formats to a sample of 262 orthopedic surgeons. Participants included orthopedic surgeons who are employed by the Ministry of Health (MOH) in Riyadh and the Eastern Province, Saudi Arabia. The questionnaire was distributed through on-site visitations to orthopedic departments in MOH hospitals as well as through online correspondence by email, WhatsApp, and social media. Results The study sample included 262 orthopedic surgeons (13.7% females and 86.3% males). Surgeons aged < 34, 35–44, and 45–54 years old represented 28.66%, 38.9%, and 20.2% of the study sample, respectively. The majority of the included surgeons did not use PROMs (69.1%), and some (17.2%) used it for research purposes. Only 5% used it regularly in daily clinical work. Conclusion The clinical use of PROMs among orthopedic surgeons was negligible, even though an overwhelming majority were interested in using PROMs. The reasons provided included a lack of knowledge on how to use PROMs and the perception that it is too time-consuming to add to regular clinical routine. There should be more efforts towards training surgeons on how to use PROMs, whereas increasing compatibility with existing software tools used by MOH hospitals may help offset time-related reservations.

Patient‐reported outcomes (PROs) are closely related to the delivery of high‐quality care and services. Patient‐reported outcome measures (PROMs) are the tools used to measure the PROs. Despite the challenges and barriers associated with using PROs and PROMs, measuring PROs is valuable for a patient's health status and advancing evidence‐based medicine. Real‐world implementation of PROMs may especially open possibilities for healthcare systems under reform, such as Saudi Arabia. This paper highlights the concepts of PROs and PROMs. Moreover, it distinguishes between PROs and other concepts such as patient experience, quality of life, and patient satisfaction. It also explores the available literature in Saudi Arabia and the world regarding PROs and their role in improving healthcare systems. Last, this paper advocates for utilizing the national and comprehensive tools of PROMS in different levels and disciplines.

Background: The current study aimed to identify the effectiveness of TKA in improving functional outcomes in a predominantly elderly patient cohort.Methods: Data were collected retrospectively from the patients admitted to a specialist orthopedic center from January 2018 to December 2019. a. Demographic and clinicopathological data and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scores at admission were acquired and entered as continuous variables to assess the outcomes in regression models values lesser than 0.05 was considered significant. Data analysis by univariate regression and Wilcoxon signed-rank test was implemented by R software (ver. 3.6.3)Results: Mean duration of operation was 90.3±29.08 minutes. Intraoperative blood loss was 91.9±26.5 ml for females and 88.6±19.1 ml for males. Mean duration of surgery was 90.3± 29.08 minutes (90.19±23.19 in patients aged ≥65 years). The mean blood loss was 334.55±114.28 mL (349.26±107.43 mL in patients aged ≥65 years). Pain, function and stiffness outcome scores decreased significantly after TKA. Post-surgery, inpatient rehabilitation was attempted by 13.89%, whilst 88.88% of individuals attempted recuperation at home. Preexisting comorbidities like diabetes mellitus was present in 11.11% individuals whilst hypertension was present in 16.67% patients. Radiographic grading revealed 5% of patients with Grade 1 Kellgren Lawrence (K-L) classification, whilst 95% of patients demonstrated Grade 4 classification.Conclusion: Understanding the underlying factors that govern TKA patients' functional restoration of mobility may reduce the length of stay. In this aspect, TKA is a viable option for improving functional outcomes even in elderly patients.
