Achieving Quality Health Outcomes Through the Implementation of a Spontaneous Awakening and Spontaneous Breathing Trial Protocol

Abstract: The duration of treatment with MV and the VUR were reduced in patients who received the SAT/SBT protocol. The incidence of self-extubation was not different when an SAT was implemented. The ICU LOS was not reduced in patients who received SATs and SBTs.

“…The evidence around DSI yields favourable patient‐related outcomes when compared to continuous infusions of sedation. Reduced mechanical ventilation days, shorter length of stay and increased survival are shown in several studies, both randomized and non‐randomized (Kress et al , ; Yiliaz et al , ; Jones et al , ; Khan et al , ). However, the use of sedation protocols and algorithms that target continuous light levels of sedation also show improved patient outcomes (de Wit et al , ; Mehta et al , ; Burry, ; Minhas et al , ).…”

“…Perante a análise dos estudos constata-se que o desmame ventilatório do doente crítico, submetido a VMI, compreende várias estratégias que podem ser organizadas em etapas consecutivas, sendo que a progressão para a etapa seguinte depende da verificação da anterior, nomeadamente: interrupção da sedação e avaliação diária do doente face aos critérios preditores do sucesso do desmame; realização de TRE e avaliação dos critérios de sucesso/insucesso do TRE; extubação (12)(13)(14)(15)(16)(17) .…”

“…Many respondents in the present study stated that delirium has a negative impact and that the ABCDE bundle could contribute to better outcomes for their ICU patients. According to previous published literature (Jones et al, 2014; Kher et al, 2013; Miller et al, 2013, 2015), even with good knowledge and effective awareness of the bundle, staff perceive that application of the bundle protocol is scarce. This result is in line with those reported by Balas et al (2013), in which only a few participants thought that the bundle was regularly applied.…”
