“…In sum, the current ACP models have not been proven effective in improving patient outcomes or aligning treatments to patient preferences; this lack of evidence was even more pronounced within vulnerable populations. Recognizing this, the American Academy of Nursing developed an ACP policy brief in 2018 highlighting obvious gaps related to ACP needs of ethnic minority, under-resourced, veterans, and lesbian, gay, bisexual, transgender, and queer (LGBTQ) Americans, in addition to persons with disabilities and residing in geographically isolated regions (Wicks et al, 2018). The American Academy of Nursing recognizes the importance of nurses at the local, state, territorial, and national level to bridge disparities in ACP (ANA Center for Ethics & Human Rights, 2016; Wicks et al, 2018). A report commissioned by the Robert Wood Johnson Foundation as background for the National Academy of Medicine committee on the Future of Nursing 2020À2030 (National Academy of Sciences, Engineering, and Medicine, 2021) has called for nurses to utilize their history, size, and well-earned trust to tackle the unmet healthcare needs of this century (American Association of Colleges of Nursing, 2019a;Pittman, 2019).…”

“…To make matters worse, difficulties in ACP completions put vulnerable populations at risk. It has been shown for years that underserved populations, including older people living alone or in poor living condition are less likely to discuss or carry out ACP, which may result in these individuals receiving end‐of‐life care that is unwanted or not aligned with their values 51–54 …”
