PURPOSE: Chemical ablation of the gallbladder may be a useful alternative to surgery for inoperable disease. The purpose of this study was to investigate the feasibility of chemical ablation of the gallbladder with acetic acid in a canine model. MATERIALS AND METHODS:Five beagle dogs underwent percutaneous transhepatic cholecystostomy. Percutaneous occlusion of the cystic duct was performed with use of n-butyl cyanoacrylate (NBCA) and coils. After confirmation of occlusion of the cystic duct, sclerotherapy of the gallbladder was performed with 4 -7 mL of 50% acetic acid through the drainage catheter. Acetic acid was retained for 20 minutes with intermittent position change. The drainage catheter was removed immediately after sclerotherapy. The dogs were euthanized 8 weeks after the procedure. The gallbladders and adjacent organs were evaluated grossly as well as microscopically.RESULTS: All dogs survived without serious complications during the experimental period. Sclerotherapy was technically successful in all dogs. Gross specimens of the gallbladder showed shrinkage and fibrotic change without retention of any bile, mucus, or pus. Histologic examinations from the body and fundus of the gallbladder demonstrated complete ablation of the mucosa. However, the neck region of the gallbladder near the cystic duct, where NBCA and coils for cystic duct occlusion were located, had focal areas of remnant or regenerating mucosa.CONCLUSIONS: Chemical ablation of the gallbladder with 50% acetic acid was effective and safe. Complete ablation was achieved in the majority of gallbladder mucosa except for a small portion located in the gallbladder neck.

ABSTRACT. This study compared percutaneous sclerotherapy using 50% acetic acid with that using 99% ethanol for patients with simple renal cysts. The study included 72 simple renal cysts in 64 patients (male/female ratio 5 31/33; age range, 31-75 years). Under fluoroscopic guidance, the cyst fluid was aspirated completely. Sclerotherapy was then performed using 50% acetic acid for 32 cysts and 99% ethanol for 40 cysts. The volumes of each renal cyst before and after sclerotherapy were compared using ultrasonography or CT. Medical records were reviewed to analyse any complications. The mean follow-up period was 21.5 months (range, 3-75 months). The mean remnant volume of the cyst after sclerotherapy was 2.6% of the initial volume in the acetic acid group and 14.0% in the ethanol group. The rates of complete remission, partial remission and treatment failure were 90.6%, 9.4% and 0%, respectively, in the acetic acid group, and 60.0%, 30.0% and 10.0%, respectively, in the ethanol group. There were no complications related to sclerotherapy in either group. In conclusion, acetic acid is a safe and effective sclerosing agent, with clinical results superior to those of ethanol, and is an alternative to ethanol for sclerotherapy of renal cysts.

Predictors of the Frequency of Ethanol Injections for Renal Cyst Ablation: A Preliminary Study

Purpose:To assess the factors facilitating the prediction of the frequency of ethanol injections in successful renal cyst ablation. Materials and Methods:We retrospectively reviewed the computed tomography and ultrasonography scans of 37 renal cysts. Two radiologists evaluated the characteristics of the presenting renal cysts, including size, calcification, septation, and lobulated configuration. Patients were divided into an "above-four" group and "below-three" group, according to the number of ethanol injections. Results: Among the 37 renal cysts, six belonged to the "above-four" group and 31 to the "below-three" group. The mean volume of "above-four" group was 409.48 cc and that of the other group was 301.64 cc. Seven renal cysts included three belonging to the "above-four" group, which showed calcification. Twelve renal cysts presented a multilocular appearance with thin septa, and included a cyst classified under the "above-four" group. Three renal cysts had lobulated configuration, with one of them belonging to the "above-four" group. Conclusion: No significant correlation was found among the different factors in the prediction of the frequency of renal cyst ablation. However, calcification was the most useful parameter for prediction of the number of renal cyst ablations needed.
