Accuracy of Sonographically Guided Intra-articular Injections in the Native Adult Hip

Abstract: Sonographic guidance can be used to inject the native adult hip joint with acceptable accuracy. When using the oblique sagittal approach, operators must be aware of the possibility of needle withdrawal from the joint due to the limited intra-articular space within the target region, particularly in the absence of effusion.

“…US-guided IA injection was performed using an anterior parasagittal approach with the freehand technique. The access to the joint cavity was detected, and the needle tip (a 23-gauge, 7.5-cm needle) was guided using the head-neck junction as a target [22][23][24]. The accuracy of injection into the IA space was checked during the procedure, and joint capsule distension was checked in appropriate cases ( Fig.…”

“…The transducer was then aligned parallel to the femoral neck, visualizing the target location within the anterior synovial recess at the femoral head-neck junction ( Fig. 1) [22][23][24]. US-guided IA injection was performed using an anterior parasagittal approach with the freehand technique.…”

“…Previous studies have utilised fluoroscopy; however ultrasound guidance has the potential advantage of speed, cost effectiveness, accurate visualisation of femoral vessels, and avoiding the use of ionising radiation and potential contrast reactions. Although the confirmation of accurate intraarticular placement of anaesthetic is less definitive than with fluoroscopy, ultrasound remains an accurate technique for guiding injections [5,6]. Our aim was to establish whether the use of diagnostic ultrasound hip joint injection of local anaesthetic can localise pathology and guide treatment in patients with possible hip osteoarthritis.…”
