We report a unique case of a 27-year-old woman with a history of cesarean section, who presented with placental invasion and omental implantation at 39 weeks + 2 days of gestation. Preoperative ultrasound did not reveal signs of placenta accreta. Despite the challenges, we successfully preserved her uterus using a combination of surgical techniques, including manual placental removal, uterine artery ligation, and repair of the uterine fundus. This case highlights the importance of a multidisciplinary approach and preoperative preparation in managing such complex obstetric complications.
