Introduction We aimed to evaluate the oesophagus of patients with reflux oesophagitis using ultrasonography and sought associations between the measurements of the mucosa, submucosa and muscularis propria, and endoscopic and clinical findings. Methods Thirty‐nine patients with endoscopically proven reflux oesophagitis and 19 patients with negative endoscopy for oesophagitis were included in the study. Two radiologists assessed the cervical oesophagus at the level of the thyroid gland. The data belonging to the ultrasonographic assessment of the patient group were compared to those of the controls. Results The muscular, submucosal and mucosal thicknesses of the cervical oesophagus measured by two observers significantly differed between the patient and control groups. The P‐values for mucosa, submucosa and muscularis propria were .02, <.001 and <.001, respectively for observer 1, .004, .001 and <.001, respectively for observer 2. The regression analysis showed that among the symptoms of heartburn, regurgitation and dysphagia, heartburn was independently associated with decreased mucosal and submucosal thicknesses of the oesophagus. Discussion Our findings confirmed that the layers of oesophagus in the gastroesophageal reflux disease were thicker compared to those of healthy people. We also found that heartburn was associated with decreased thicknesses of mucosal and submucosal layers in patients.

Background: Neoadjuvant chemotherapy (NAC) has been proven to effectively improve the prognosis and long-term survival of patients with esophageal cancer (EC). But approximately 40% of patients are relatively insensitive to NAC. The mechanism underlying gene-induced resistance remains elusive. Methods: We conducted a cohort of 13 NAC patients with different chemotherapy responses to identify gene mutations related to drug resistance by next-generation sequencing (NGS) of samples from patients with EC. We performed protein conformation on these mutant genes to figure out the possible mechanisms related to resistance. Results: Our results indicated that missense mutations were commonly emerged in patients with partial response (PR) and stable disease (SD). Moreover, NOTCH1 gene, which is closely related to efficacy of chemotherapy, was further screened by comparing the changes in gene mutations before and after chemotherapy. Through protein conformational analysis, we found that missense mutations may cause changes in the ability of NOTCH1 receptor protein to bind ligands, which may cause abnormalities in the NOTCH1 pathway and make patients resistant to chemotherapy. Conclusions: We analyzed the effect of platinum-based NAC on gene mutations in patients with EC and find that mutations in somatic genes, especially the NOTCH1 gene, may be associated with NAC resistance in EC.

Prognostic Value of CT Imaging-Based Tumor Volume in Patients With Non-Surgical Esophageal Squamous Cell Carcinoma

BackgroundThe American Joint Committee on Cancer-Tumor (AJCC-T) staging system for esophageal carcinoma patients, which is based on the depth of tumor invasion, is not applicable in some cases. This study aims to assess the prognostic value of CT imaging-based tumor volume and its usefulness for T staging in patients with non-surgical esophageal squamous cell carcinoma (ESCC).MethodsWe retrospectively reviewed the records of 158 ESCC patients undergoing definitive (chemo) radiotherapy from two hospitals. Tumor volume based on the CT imaging was calculated using the formula: V = πabc / 6. Three cutoff points for tumor volume were obtained with the X-tile software. Overall survival (OS) was analyzed using the Kaplan–Meier method. The -2 log-likelihood ratio and Akaike Information Criterion (AIC) value were evaluated to compare the AJCC-T staging system with the proposed T staging method.ResultsThe median tumor volume was 19.8 cm³ (range from 1.0 to 319.5 cm³). The three optimal cutoff points of tumor volume were 12.7, 22.8, and 51.9 cm³, and the patients were divided into four groups named as proposed T1–T4 stages. The 3-year OS rates in patients with proposed T1 to T4 stages were 67.9%, 30.6%, 21.3%, and 5.3%, respectively. The −2 log-likelihood ratios of the AJCC-T stage and proposed T stage were 1,068.060 and 1,047.418, respectively. The difference in the AIC value between the two T staging systems was 18.642.ConclusionCT imaging-based tumor volume was superior to the depth of tumor invasion for T staging in predicting the prognosis of non-surgical ESCC patient.
