Accuracy of Doppler Echocardiography in the Hemodynamic Assessment of Pulmonary Hypertension

Abstract: Rationale: Transthoracic Doppler echocardiography is recommended for screening for the presence of pulmonary hypertension (PH). However, some recent studies have suggested that Doppler echocardiographic pulmonary artery pressure estimates may frequently be inaccurate. Objectives: Evaluate the accuracy of Doppler echocardiography for estimating pulmonary artery pressure and cardiac output. Methods: We conducted a prospective study on patients with various forms of PH who underwent comprehensive Doppler echocard… Show more

“…Nonetheless, a recent study in patients with PH (mPAP >25 mm Hg) demonstrated modest correlation and poor agreement between echocardiography‐derived and RHC‐measured PASP and mPAP 28. Fisher et al29 showed that approximately half of the cases of PASP overestimation are related solely to right atrial pressure overestimation by echocardiography. Thus, although we used an assumed right atrial pressure of 5 mm Hg for calculating PASP in echocardiography studies that reported only tricuspid regurgitation velocity or gradient, concern for misclassification remains when relying on echocardiography alone for the diagnosis of PH (or mild PH).…”

“…We elected not to add estimates of right atrial pressure to the TRPG in this study, as consensus is lacking on how best to do so in dogs and adding right atrial pressure to TRPG only minimally improved accuracy of estimating invasive systolic PA pressure in a recent experimental study of acute PH in dogs 9. There is also evidence in humans that adding estimations of right atrial pressure to estimate systolic PA pressure can result in overestimation of PH severity 27. Also, compared to 2 previous studies in dogs that evaluated diagnostic accuracy of AT, AT:ET, and MPA:Ao in predicting PH (ie, echocardiographic evidence of at least mild PH was present),10, 11 similar to slightly improved sensitivity, specificity, and AUC values were obtained.…”

“…This approach is known to be unreliable, with high rates of false positives and false negatives (9,10). Nevertheless, with baseline echocardiography screening, 33 of 578 SSc patients in the ItinĂ©rAIR-SclĂ©rodermie study were identified as having possible PAH.…”
