“…With recent technological advancements and development of multidetector CT, CT angiography (CTA) has been increasingly found to be as effective or even superior to traditional angiography, not only in providing the necessary information for diagnosis and management in a rapid and non-invasive manner, but also for disclosing the bony clinoidal anatomy, which is useful during surgery. 18,19 Balloon test occlusion, with or without hypotensive challenge, may be needed to evaluate the patient's tolerance for carotid occlusion as a definitive therapy and to assess the need for a bypass procedure. A bypass may be required for giant aneurysms 10,12,17 or for those in which the wall is extremely friable.…”

“…subarachnoid hemorrhage. The sensitivity of CTA for detection of aneurysms ranges in recent studies with 16-slice CT between 92.5% and 98.4% (3,12,13). However, in the study by Yoon et al (13) sensitivities as low as 74.1% and 77.8% (two readers) for aneurysms ,3 mm were found, showing the difficulties to detect these subtle but crucial pathologies with CTA.…”

“…This was an unusual case and the CTA was not sufficient to rule out vascular pathology. CT angiography has a reported sensitivity between 91 and 100% depending on the aneurysm size,[4524] and formal angiography needs to be performed in a setting such as this where suspicion is high. Without a perfect match between clinical presentation and imaging, more initial investigation would have been useful and could have prevented a nearly catastrophic event.…”
