Accumulation of technetium-99m pertechnetate in a patient with metastases of thyroid carcinoma

Abstract: Accumulation of both Tc-99m pertechnetate and radioiodine upon scintigraphy in thyroid carcinoma and/or in its metastases is a rare occurrence. In this paper we describe a patient who was taken to surgery for left lobectomy of the thyroid with follicular adenocarcinoma and who had accumulation of both I-131 and Tc-99m pertechnetate in lung metastases. The accumulation of I-131 was less than that of Tc-99m pertechnetate. The use of Tc-99m pertechnetate for imaging for diagnosis of functioning thyroid metastases… Show more

“…Differentiating a benign follicular adenoma from a malignant follicular carcinoma is challenging by cytology, and a thyroid scan is advocated in these cases, considering functioning nodules as being benign [1, 21]. Hot nodules outside the thyroid can be helpful in diagnosis of malignancy in the case of metastatic thyroid carcinoma, but this is rare in practice [22]. In our patient, surgical treatment was preferred to radioiodine ablation considering her symptoms and the nodule's size.…”

“…Most studies of 99m Tc-pertechnetate scans in patients with DTC mainly focused on the diagnostic efficiency of thyroid remnant or predicted value of lowto intermediate-risk DTC's therapy response to RAI therapy [7][8][9]. In postoperative 99m Tc-pertechnetate scan, 99m Tcpertechnetate avidity of DTC metastatic lesions is a rare phenomenon, only described by case reports [10][11][12][13][14][15], in which the 99m Tc-pertechnetate-avid metastatic foci were detected serendipitously. Some literature described the clinical characteristics of 99m Tc-pertechnetate-avid metastatic foci in DTC, including distant metastasis like lung and bone [16][17][18].…”

“… 6 Over the last few decades, extra-thyroid 99m Tc-pertechnetate uptake has rarely been reported. 7 , 8 Furthermore, almost all available literature includes case reports, 9 14 most of which confirmed DTC metastases.…”
