Accumulation of Iodized Oil Within the Non-Neoplastic Liver Adjacent to Hepatocellular Carcinoma via the Drainage Routes of the Tumor After Transcatheter Arterial Embolization

Abstract: The drainage of blood from the HCC was considered to be a possible mechanism of the accumulation of Lipiodol in the nontumorous liver adjacent to the HCC.

“…The lipiodol is also carried to non-cancerous parenchyma surrounding the tumor via accompanying the portal veins through the peribiliary arterial plexus and/or peri-tumoral portal vessels drained through the blood sinusoid of the tumor. Consequently, higher incidence of complete necrosis is speculated to occur which is confirmed by CT and pathological examination (16,42). It is presumed that calibrated beads such as drug-eluting beads (DEBs) and EmboSpherew available in Western countries do not show the similar phenomenon as seen in lipiodol.…”

“…Continuous infusion of 5-FU may enhance the antitumor effect in cisplatin-lipiodol plus 5-FU therapy compared with other HAIC regimens. The antitumor effect has been reported to be more potent with anticancer agents in lipiodol suspension than with anticancer agents alone, such as sorafenib (42).…”
