Access to the skull base - Maxillary swing procedure - Long term analysis

Abstract: Background:Skull base is difficult to approach surgically due to its complex anatomy. A number of procedures that is endoscopic, microscopic, and open approaches have been used. The maxillary swing approach provides a wide exposure to the surgeon for better oncological clearance.Patients and Methods:A total of 62 patients with varied etiologies involving the skull base region were operated with maxillary swing procedure over a period of 15 years from 2001 to 2016 in plastic surgery department at a single insti… Show more

“…The PPF is a relatively small and concealed area that communicates with intracranial and extracranial compartments via multiple bony canals and foramina through which different neoplasms can spread back and forth ( 5 ). The heterogeneity of the PPF’s tissues makes it a bed for a wide spectrum of benign and malignant lesions with variable prognoses ( 8 ), but its anatomical complexity poses a challenge to surgeons who hesitantly commit to removing such lesions.…”

“…Several variations of transfacial swing procedures exist and have been comprehensively reviewed by Dr. Ian Jackson 20 with excellent long-term outcomes reported by Kalra and colleagues. 28 Transmaxillary: Two-Piece Lefort I Through an upper buccal sulcus incision, the maxilla is exposed as for a Lefort I osteotomy. Intraorally, the palate is incised in the midline, exposed, and osteotomized.…”

“…In presurgical planning, attention is paid to the hand-dominance of the resecting neurosurgeon; for right-handed surgeons standing at the head of the bed, the maxilla is swung to the patient's left and the opposite for left-handed surgeons. Several variations of transfacial swing procedures exist and have been comprehensively reviewed by Dr. Ian Jackson 20 with excellent long-term outcomes reported by Kalra and colleagues 28 …”

“…The total resection rate varies from 57% to 77%. [1][2][3][4] Surgical treatment of complex skull base tumors should be the first choice. [5][6][7] Tumor location, size, nature, and surrounding structures are significant to be evaluated comprehensively.…”
