Vaccination against SARS-CoV2 is the most important weapon in the arsenal in the battle against COVID-19. There is concern about an increased risk of adverse effects in patients with transfusion-dependent thalassemia (TDT), which affects vaccine acceptance. A predesigned questionnaire was used to evaluate adverse effects (local/systemic within 90 days after vaccination) in participants >18 years of age with TDT. A total of 100 patients received 129 vaccine doses. The mean age of the patients was 24.3 ± 5.7 years and M: F ratio of 1.6:1. Covishield (Serum Institute of India) was administered to 89% of the participants and Covaxin (Bharat Biotech Limited) to 11% of the participants. Adverse effects were documented in 62% of the respondents and were more pronounced after the first dose (52%) compared to the second dose (9%). The most frequent adverse effects were pain at the injection site (43%) and fever (37%). All adverse effects were mild and none of the participants required hospitalization. There were no differences in adverse effects among different vaccines, the presence or absence of comorbidities, blood groups, or ferritin levels. The SARS-CoV2 vaccine appears to be safe in patients with TDT.

OBJECTIVES: Omicron was declared as a variant of concern by WHO on 26 Nov 2021. Omicron is highly transmissible, but the disease severity and morbidity were lesser compared to the Delta variant. However, COVID-19 Vaccine efficacy was reduced for the Omicron variant whereas it was highly efficacious for the Delta variant. Hence, for evidence-based counseling in pregnant patients about expected outcomes depending on their vaccination status, this prospective cohort study was conducted. STUDY DESIGN: This study was conducted in Base Hospital Delhi Cantt, New Delhi, India during the third wave of SARS-CoV-2 i.e. from Jan 2022 to Mar 2022. All COVID-19-positive patients who were admitted for delivery were followed up till discharge from the hospital. The outcomes in terms of severity of COVID-19 infection, period of gestation at the time of delivery, intrapartum/postpartum complications, fetal distress, meconium staining of liquor, the requirement of neonatal intensive care unit admission were documented and data was analyzed to assess clinical severity of the disease in fully/partially vaccinated+unvaccinated women. RESULTS: During the specified period, 22.32% was the positivity rate among the delivered patients. Of 61.78% were fully vaccinated and 24.39% were either not vaccinated or partially vaccinated. The risk of symptomatic COVID-19 illness, the requirement of supportive management, and maternal and neonatal outcomes in both groups were comparable. CONCLUSION: Unvaccinated or partially vaccinated parturient had no increased risk of symptomatic COVID-19 illness or requirement of supportive management in terms of oxygen inhalation or ventilation as compared with fully vaccinated pregnant women. The study also reported comparable maternal and fetal outcomes in vaccinated and unvaccinated/ partially vaccinated pregnant women. Further studies are required to ascertain whether the comparable outcomes were due to the decreased severity of the disease caused by the omicron variant.

The ongoing global COVID-19 pandemic has infected hundreds of millions of people over the world, imposing a tremendous burden on the global healthcare system including the dental community. COVID-19 vaccines are currently the best defense against the rapidly evolving severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). However, there are emerging concerns about vaccine breakthrough infections. This study has been done to assess the COVID- 19 breakthrough infection among the vaccinated Dental student community of North Kerala and also evaluate and compare the severity of COVID-19 infection among vaccinated and non-vaccinated individuals. A survey was performed within the Dental student community which included BDS students, House surgeons and Post graduates by means of a questionnaire administered online via Google forms that consisted of demographic information and questions to assess severity of the COVID-19 breakthrough infection. The data were analyzed using the SPSS version 26 software. Result was presented in frequencies, percentages and charts showing the interrelatedness of the variables examined. Among the 714 respondents there were 11.9% males and 88.1% females. Majority of the study subjects reported to have completed two doses of Covid vaccine, while 11.5% had taken only a single dose and 1% had not been vaccinated at all. Around 13% of subjects gave history of being Covid positive before vaccination and (14.7%) got infected even after vaccination.New Covid variants are being evolved continuously, thereby increasing the chance for transmissibility and evasion of the host immune response. This could be the reason for the 14.7% breakthrough infections reported in our study. Therefore, the need for future studies to recognize factors leading to inadequate vaccine response in those with breakthrough infections and ways to mitigate them is highlighted.
