Accelerated workflow for primary jaw reconstruction with microvascular fibula graft

Abstract: IntroductionMajor facial defects due to cancer or deformities can be reconstructed through microvascular osteocutaneous flaps. Hereby CAD/CAM workflows offer a possibility to optimize reconstruct and reduce surgical time. We present a retrospectiv observational study regarding the developement of an in-house workflow allowing an accelerated CAD/CAM fibula reconstruction without outsourcing.Case descriptionWorkflow includes data acquisition through computertomography of head and legs, segmentation of the data a… Show more

“…The computeraided design/computer-aided manufacturing workflow for modeling allows the preoperative definition of cutting paths and angles at the resection site and of the graft, as well as the shape of the osteosynthesis material. 185,186 This process can either be done via commercial platforms or the clinic itself, depending on the available resources. On the one hand, this increases accuracy and reduces operation times.…”

“…The devices generated may be divided into three main categories: (i) Pre-surgical tools, (ii) intrasurgical tools, and (iii) implants[ 1 ]. At present, some of the possible applications include individualized joint prosthesis, spine stabilization, individual osteosynthesis, and reconstructions or surgical accessories such as drilling or cutting guides[ 2 - 10 ]. Even the combination of a printed surgical guide and a printed dental implant with a modified surface could be possible[ 11 ].…”

“…In secondary mandibular defect reconstruction, preoperative CT angiography of the neck is also considered necessary for surgeons to be better prepared for the difficulties they will encounter. Goetze et al [ 25 ] refer the loss of two fibula free flap in seven patients and other researchers stress that most complications for this kind of flap involve vascular compromise [ 3 , 26 ]. Preoperative angiography or 3D CT images of the neck are mandatory to select the most suitable recipient vessels according to its location and diameter [ 3 , 27 ].…”
