Background Adaptive radiotherapy is introduced in the management of urinary bladder cancer to account for day-to-day anatomical changes. The purpose of this study was to determine whether an adaptive plan selection strategy using either the first four cone beam computed tomography scans (CBCT-based strategy) for plan creation, or the interpolation of bladder volumes on pretreatment CT scans (CT-based strategy), is better in terms of tumor control probability (TCP) and normal tissue sparing while taking the clinically applied fractionation schedules also into account. Material and methods With the CT-based strategy, a library of five plans was created. Patients received 55 Gy to the bladder tumor and 40 Gy to the non-involved bladder and lymph nodes, in 20 fractions. With the CBCT-based strategy, a library of three plans was created, and patients received 70 Gy to the tumor, 60 Gy to the bladder and 48 Gy to the lymph nodes, in 30-35 fractions. Ten patients were analyzed for each adaptive plan selection strategy. TCP was calculated applying the clinically used fractionation schedules, as well as a rescaling of the dose from 55 to 70 Gy for the CT-based strategy. For rectum and bowel, equivalent doses in 2 Gy fractions (EQD2) were calculated. Results The CBCT-based strategy resulted in a median TCP of 75%, compared to 49% for the CT-based strategy, the latter improving to 72% upon rescaling the dose to 70 Gy. A median rectum V30Gy (EQD2) of 26% [interquartile range (IQR): 8-52%] was found for the CT-based strategy, compared to 58% (IQR: 55-73%) for the CBCT-based strategy. Also the bowel doses were lower with the CT-based strategy. Conclusions Whereas the higher total bladder TCP for the CBCT-based strategy is due to prescription differences, the adaptive strategy based on CT scans results in the lowest rectum and bowel cavity doses.

Bevezetés és célkitűzés:Lipiodollal jelölt tumorágy alapján végzett képvezérelt sugárkezelés, illetve szimultán integrált boost technika bevezetése izominvazív hólyagdaganatok radiokemoterápiájában. Módszer: Izominvazív tranziciocelluláris hólyagcarcinoma miatt 2016. április óta három férfi betegünknél végeztünk radiokemoterápiát. A radiokemoterápia megkezdése előtt mindegyiküknél tumorágy-reszekciót végeztünk, amely során a daganatágyba submucosalisan 10 ml lipiodolos oldatot fecskendeztünk be, így jelölve a tumor helyét az emelt dózisú besugárzáshoz. A radiokemoterápia során 30 frakcióban a kismedencére 51 Gy-t (napi 1,7 Gy), a teljes hó-lyagra 57 Gy-t (napi 1,9 Gy), a lipiodollal megjelölt tumorágyra 63 Gy-t (napi 2,1 Gy) adtunk szimultán integrált boost formájában, forgóíves, intenzitásmodulált technikával. A besugárzás pontosságát napi kilovoltos CT-vel biztosítottuk. A radiokemoterápia alatt jelentkező korai radiogén urogenitalis és gastrointestinalis mellékhatásokat a Radiation Therapy Oncology Group beosztása szerint osztályoztuk. Eredmények: A lipiodol beadása során és azt követően érdemi perioperatív mellékhatást, toxicitást nem észleltünk. Betegeinknél az előírt dózist leadtuk. A sugárkezelés időtartama hat hét volt (heti öt frakció). A SIB-kezeléshez kialakított dózis-térfogati megszorításokat minden betegünknél teljesítettük. A kilovoltos CT-ellenőrzések során a lipiodollal megjelölt tumorágy jól látható volt. A kezelés alatt egy betegnél grade II cystitist és grade II proctitist, egy betegnél grade I cystitist észleltünk, amelyek tüneti, gyógyszeres kezeléssel javultak. A harmadik betegnél akut radiogén mellékhatás nem alakult ki. Következtetések: A lipiodol hólyagfali injektálása biztonságos volt, perioperatív toxicitást nem okozott. A tumorágy lipiodolos jelöléssel jól látható volt a sugárkezelés előtt végzett kilovoltos CT-n. A teljes kezelési idő négy nappal megrövidült, a tumorágyra leadott biológiai összdózis emelése mellett. A korai radiogén mellékhatások mérsékeltek voltak. Radykalna cystektomia jest leczeniem z wyboru u chorych na inwazyjnego, naciekającego mięśniówkę właściwą raka pęcherza moczowego. Obok chirurgii teleradioterapia stanowi podstawową metodę radykalnego leczenia chorych na inwazyjnego raka pęcherza moczowego. Skojarzone leczenie zachowawcze: TURB, radioterapia i chemioterapia, pozwala na uzyskanie 5-letnich przeżyć całkowitych, wahających się od 50% do 63%, przy zachowaniu u około trzech czwartych chorych prawidłowo funkcjonującego pęcherza. Wyniki radykalnego zachowawczego leczenia miejscowego: TURB z następową samodzielną teleradioterapią są niezadowalające, a 5-letnie przeżycia całkowite wynoszą od 20% do 40%. Prowadzone są liczne badania kliniczne mające na celu poprawę skuteczności napromieniania chorych na inwazyjnego raka pęcherza moczowego poprzez: podwyższanie całkowitej dawki, modyfikowanie obszaru napromienianego, stosowanie niekonwencjonalnych metod frakcjonacji, stosowanie hipertermii, podawanie środków radiouczulających. Samodzielna teleradioterapia jako leczenie radykalne inwazyjnego raka pęcherza moczowego jest obecnie zalecana jedynie w przypadku chorych niezakwalifikowanych do leczenia operacyjnego lub niewyrażających zgody na operację, równocześnie niekwalifikujących się do leczenia systemowego. Biorąc pod uwagę fakt, że rak pęcherza moczowego jest nowotworem pacjentów w starszym wieku, jak również ciągłe starzenie się naszej populacji, samodzielna radioterapia może stanowić jedyną formę leczenia stwarzającą szansę na wyleczenie z inwazyjnego, naciekającego raka pęcherza moczowego w stosunkowo licznej grupie chorych. Rola uzupełniającej radioterapii w stosunku do radykalnej i częściowej cystektomii wymaga oceny w wielośrodkowych randomizowanych badaniach klinicznych. Role of external radiation therapy in invasive bladder cancerRadical cystectomy is the treatment of choice for patients with muscle-invasive bladder cancer. Radiotherapy, as well as surgery are essential methods of treatment of invasive bladder cancer. Combined modality bladder-sparing treatment can achieve 5-year overall survival rates of 50-63%, and with some 75% of surviving patients maintaining their bladder. For local treatment alone, radiation therapy following TURB, has resulted in 5-year survival rates of 20-40%. Many studies have been performed to improve the results of radiotherapy in the treatment of invasive bladder cancer by escalation of total dose, adoption of altered fractionation, modification of treatment volume, application of hyperthermia or using radiosensitizers. Radical radiotherapy following TURB should be used only in patients who are not surgical candidates or who have refused surgery and are not suitable for chemotherapy. Since bladder cancer is a disease of an elderly and aging population, definitive radiotherapy can be only one possibility for radical treatment for relatively large number of patients with muscle-invasive bladder cancer.
