Background Surgical-assisted accelerated orthodontics (SAAO) has become very popular recently. Therefore, this study aimed to investigate the extent to which researchers adhere to Item 19 (harms) of the Consolidated Standards of Reporting Trials (CONSORT) in the published studies in the field of SAAO. In addition, the study evaluated the possible association between harm reporting and the human development index (HDI) of the recruited research sample country, CiteScore-based quartile (CSBQ) of the publishing journal, invasiveness of the surgical intervention (ISI), and the type of orthodontic tooth movement (TOTM). Moreover, it aimed to summarize the different possible harms and complications that maybe encountered in the course of SAAO. Materials and methods Electronic searching of six databases was conducted for SAAO-related English RCTs published between January 2000 and April 2022. For the RCTs that did not report harms, information was sought by contacting the corresponding authors. Descriptive statistics of the evaluated variables were performed. The association between 'harm reporting' and the HDI of the research team, the BDRQ of the publication journal, the ISI, and the TOTM were investigated. Binary logistic regression was used, and the odds ratios (ORs) with 95% confidence interval (CIs) of the evaluated variables were obtained. Moreover, the risk of bias of the included RCTs was assessed using the RoB2 tool. Results Among the 91 included RCTs, 54 RCTs (59.3%) did not adhere to reporting harm associated with the SAAO. The non-adherence was significantly associated with the ISI (OR 0.16; CI 0.03–0.73; p < 0.018) for invasive methods compared with minimally invasive ones). There was a significant positive correlation between harm reporting and both the CSBQ of the publishing journal and the HDI of the recruited research sample country (p = 0.001, p = 0.003, respectively). On the contrary, a non-significant association was found between harm reporting and the type of OTM (p = 0.695). The incidence of harms associated with SAAO was approximately 17.5%. Limitations Assessment was restricted to English RCTs related to SAAO. Conclusion and implications The adherence to reporting harms in the field of SAAO was deficient. Efforts should be made by authors, peer reviewers, and editors to improve compliance with the CONSORT guidelines regarding harms reporting. Additionally, there is a wide spectrum of harms that could be associated with SAAO that the practitioner should pay attention to and alert the patient to the possibility of their occurrence.

Due to the overwhelming desire among adults for shorter orthodontic treatment times, there is a growing trend in research that focuses on accelerating procedures for tooth movement. Unfortunately, lengthy orthodontic treatment times come with a number of adverse effects, including an increased risk of tooth decay, gingival recession, and root resorption. Finding the greatest way to maximize tooth motion with the least drawbacks is now more important than ever. The surgical method provides the most dependable results, but its invasiveness limits its use. Corticotomy is one of the commonly used techniques in practice for acceleration of orthodontic tooth movement. Numerous corticotomy methods have been successful in causing accelerated tooth movement. Through the use of these approaches, local acceleratory phenomena are activated, fostering an ideal milieu for tooth movement acceleration. In order to expedite orthodontic tooth movement while adhering to a specified surgical and orthodontic regimen, the piezocision treatment appears to be the ideal compromise. In cases of dehiscence and/or fenestration on the alveolar bone linked to moderate to severe overcrowding, piezocision surgery enables the inclusion of biomaterials. Low-level laser therapy and orthognathic surgery-first are also methods used to accelerate tooth motion among various others. However, further research should be done to ascertain the optimal way to accelerate tooth movement because each approach has benefits and drawbacks. Objectives No study has compared pain intensity and pulp vitality of both activation protocols of periodontal distractor (twice and four activations per day) for rapid canine retraction. Since the periodontal distractor may cause some degree of suffering for the patients and may affect pulp vitality, it is important for orthodontists to handle this situation in the best possible way by choosing the most effective and the least painful method of treatment. Therefore the aim of the present study was to evaluate the effect of twice and four activations/day of periodontal distractor on pain intensity and pulp vitality of maxillary canine. Methods The study was carried out on 32 canine of 16 female patients (16–22 years of age) requiring extraction of bilateral maxillary first premolars and maximum anchorage. They were divided into two groups. (Group I): Canine retraction was done by periodontal distractor with twice activations per day. (Group II): Canine retraction was done by periodontal distractor with four activations per day. Periodontal distractor was cemented after bilateral maxillary premolar extraction and inter septal bone corticotomy was done. Pain intensity was evaluated at different times by Visual Analogue scale (1st 48 hour after intervention, during each activation of the distractor, and on the Interval between activation).Pulp vitality was measured by electric pulp tester. Results Comparison of pain intensity between different intervals was performed by using Friedman`s test (non-parametric data) which revealed significant difference in both groups, as 1st 48 hours was significantly the highest (8.8 ± 0.77), (9.2 ± 0.77) followed by During each activation (5.4 ± 0.94), (6.9 ± 1.25), while Interval between activation (1 ± 1.03), (2.7 ± 1.38) regarding group I and II respectively. Comparison between both groups was performed by using Mann Whitney`s test (non-parametric), which revealed that there was significant difference during each activation and intervals between activations as group II was significantly higher than group I (P = 0.0001).None of the teeth reacted negatively to the electrical vitality test that was performed one month after the completion of the distraction procedure. There was no clinical sign of discoloration or pulpal pain in any tooth. Conclusion In this study, Pain intensity was y significantly higher in group II (four activations/ day) than group I (twice activations/ day). The pulp of the retracted canine teeth examined in this study for both groups was vital on both groups.
