Accelerated Degeneration of a Bovine Pericardial Bioprosthetic Aortic Valve in Children and Young Adults

Abstract: Background-Experience with aortic valve replacement (AVR) with current-generation pericardial bioprostheses in young patients is limited. The death of a child with accelerated bioprosthetic aortic stenosis prompted enhanced surveillance of all such patients at our institution. Methods and Results-We reviewed records of 27 patients who had undergone AVR (median follow-up, 13.7 months) with a bovine pericardial bioprosthesis at ≤30 years of age. In the Mitroflow LXA valve group (n=15), freedom from valve failure… Show more

“…In the report that soon followed, these authors described their experience with early failure of the Mitroflow valve in the aortic position in patients aged 30 years or younger [11]. The authors concluded that this particular bioprosthesis should not be used for aortic valve replacement (AVR) in the young.…”

“…Many studies have proven that in the young, bioprostheses are prone to accelerated degeneration [8]. Follow-up of young patients with Carpentier-Edwards pericardial bovine valve implants show their long-term durability with low rates of structural failure [9]. It should be emphasized that rigorous post-operative surveillance is critical for early detection of possible complications and their prompt management.…”

“…This was defined by an increase in mean valvular gradient >25 mm Hg/y. Recently, Saleeb et al 14 have reported this accelerated degeneration of the Mitroflow valve in a young patient population (<30 years of age). Freedom from valve failure was 53% at 2 years and 18% at 3 years in this series.…”
