Sequential Follow-Up Results of Catheter Intervention for Coronary Artery Lesions After Kawasaki Disease

Background-The purpose of this study was to assess the sequential follow-up results of catheter intervention in Kawasaki disease by use of quantitative coronary angiography (QCA) and intravascular ultrasound imaging. Methods and Results-Catheter intervention was performed on 23 stenotic lesions in 22 patients (aged 2 to 24 years).Percutaneous balloon angioplasty (PBA) was performed in 4 patients, stent implantation in 7, percutaneous transluminal coronary rotational ablation (PTCRA) in 10, and a combination of PTCRA with stent implantation in 2. A total of 21 lesions (91%) were successfully dilated by catheter intervention without major or minor complications. One patient immediately underwent coronary artery bypass grafting (CABG) surgery because stent implantation failed to resolve his lesion. At 4 to 6 months after catheter intervention, 2 restenotic lesions (9%) were detected by QCA in 2 patients who had undergone PBA, and these patients subsequently underwent CABG surgery. In 6 months to 3 years after catheter intervention, no patients showed evidence of ischemic findings. At 3 to 4 years after catheter intervention, QCA and intravascular ultrasound studies were performed on 15 lesions in 14 patients. Two restenotic lesions (13%) were detected by QCA in 2 patients. One of the 2 had stent implantation and underwent CABG surgery, and the other had undergone PTCRA and underwent re-PTCRA. Thirteen patients demonstrated no ischemic findings at 3 to 8 years after catheter intervention. Conclusion-Catheter intervention for Kawasaki disease can be accomplished and can be effective in the short term, but the long-term efficacy should be verified by further study. IntroductionWe investigated the outcomes of stenting with overlapping drug-eluting stents (DES) versus overlapping stenting with a combination of drug-eluting and bare metal stents (BMS) in very long ≥(≥ 25 mm).Methods and ResultsFifty-two patients treated with either overlapping DES-DES (n = 22) or DES-BMS (n = 30) were selected from a registry of 588 patients with very long coronary lesions. Patients with acute myocardial infarction (MI) within the preceding 48 hours were excluded. The DES-DES combination was more frequently used for longer lesions compared with the DES-BMS group (47.95 ± 9.25 vs 39.98 ± 9.15 mm, p = 0.003). Left anterior descending artery lesions were also more frequently treated with the DES-DES combination (95.5 vs 66.7%, p = 0.02). In four patients in the DES-BMS group, overlapping stents were used for the coverage of dissections. Peri-procedural non-Q-wave MI occurred in one patient in the DES-BMS group. On follow up, only one case of non-fatal MI occurred in a patient with overlapping DES-DES.ConclusionOverlapping a BMS in the proximal part of a long DES instead of exclusive deployment of two or more overlapped DES seems to be a safe and feasible therapeutic strategy in our practice.

This is an open non-randomized study designed to evaluate clinical and angiographic shortand long-term results of the EPHESOS stent implantation in 457 patients (M/F=348/109) with unstable angina (UA) or stable angina (SA) and native lesions. In SA group 268 stents were implanted in 231 patients. In UA group 271 stents were implanted in 226 patients. 46% of lesions were complex. The length of stenosis was 12.9±6.7 mm in SA group, and 14.1±7.4 mm in UA group. The rate of long stenosis was 30%. Coronary Stenting success rate appeared to be 99% of cases free from acute thrombosis. 1.3% of patients in UA group suffered a non-fatal myocardial infarction (MI) during the in-hospital period. Major adverse cardiac events (MACE) rate including mortality rate and the occurrence of recurrent angina, MI and restenosis as well as repeat revascularization during 6-month follow-up, was 15.6% in SA group and 18.1% in UA group. Angiographic 6-month control revealed the loss index of 0.22±0.20 in SA group and 0.30±0.27 in UA group.18-month follow-up did not reveal any significant differences in MACE rate between the groups (21.6% for SA and 22.6% for UA group). These results indicate the efficacy and safety of EPHESOS stent with reasonable rates of stent thrombosis and restenosis in SA and UA patients undergone high risk endovascular interventions.
