Objective Acanthamoeba keratitis cause severe corneal infection and lead to poor vision and blindness. This disease is caused by a unicellular amphizoic protozoon called Acanthamoeba spp. that present in different environments. This study aimed to represent the existence and genotyping of Acanthamoeba spp. in patients with keratitis and swimming pool water (SPW) in Tehran Province, Central Iran. Methods In this descriptive study, 56 clinical samples were collected from patients with keratitis and 30 water samples were collected from different swimming pools in Tehran Province. All samples were examined based on the morphological and molecular techniques. The genotypes were determined by sequencing the partial of 18S rRNA gene. Results Of 56 clinical (corneal) and 30 environmental (SPW) samples, 30.3% and 40.0% were positive for Acanthamoeba spp., respectively. According to sequencing analysis, 94.1% of amoebic keratitis isolates were belonged to T4 genotype and only one (5.8%) isolate was belonged to T11 genotype. All genotypes were detected from SPW samples were identified as T4 genotype. Conclusion According to our results, use of contact lens and swimming in pool poses the major risk factor for amoebic keratitis in the studied area (Tehran). Moreover, T4 genotype was the predominant genotype of human keratitis and swimming pool samples there. Consequently, essential and practical measures are urgently needed to prevent subjects against this ocular seriously disease.

Introduction and purpose Infectious keratitis represents the leading cause of corneal blindness worldwide, particularly in developing countries. Both risk factors and etiological patterns vary between economically developed and developing regions, with bacterial predominance in the former and fungal predominance in the latter. This review aims to provide an updated overview of the risk factors in relation to infectious keratitis. Description of the state of knowledge Infectious keratitis is a painful and potentially vision-threatening ophthalmic condition characterized by decreased vision, photophobia and eye redness. In severe cases, it may require hospital admission for intensive medical treatment or surgical interventions. It can be caused by a wide array of organisms, including bacteria, fungi, viruses, parasites or polymicrobial infection. The most commonly observed predisposing factors for infectious keratitis include contact lens wear, trauma, ocular surface diseases, post-corneal surgery and systemic or local immunosuppression. Manual workers in construction or agricultural industry, exposed to vegetative matter, organic materials and animal products are particularly at risk. Higher rate of incidence is also observed in regions with poor environmental and personal hygiene, low level of education, poor access to sanitation and healthcare facility. Summary Persistent prevalence of infectious keratitis in both developed and developing countries makes it a serious medical problem. Prevention, proper diagnosis and effective treatment are crucial for positive visual outcome.

Background: Dry eye (DE) is a multifactorial disorder that can influence tear production, functional visual acuity and ultimately increase the osmolarity of the tear film. The prevalence of DE ranges from 7% to 33% across the world. However, to the best of our knowledge, the prevalence range of DE in Arab countries is not precisely documented in the literature.Aim: The aim of this article was to determine the prevalence range of DE, investigate the major risk factors of DE and identify the clinical diagnosis and medical management of DE.Method: In this study, only English language articles from 2017 to 2020 were selected. There were 52 articles on prevalence, risk factors, clinical diagnosis and medical management of DE in the Arab population.Results: The prevalence of DE in the Arab population varies in reports, from 10% in the United Arab Emirates (Dubai) to 69% in Palestine (West Bank). Gender difference (DE more in women), wearing of contact lenses, diabetes mellitus and glaucoma were all known to intensify the symptoms of DE. Lastly, there are two approaches to reduce DE in the Arab population, namely, pharmacologic and non-pharmacologic methods.Conclusion: The prevalence of DE in the Arab population was relatively high. In addition, the ocular surface disease index is one of the most common tools for the diagnosis of DE, whilst tear break-up time test is the common clinical test used in the Arab reports. Lastly, the most common treatment for DE is artificial tears.
