Delirium is a major complication most commonly observed in patients with advanced cancer. However, despite its prevalence, the early diagnosis, management, and prevention of this condition have not seen significant progress. Aim of this research is to provide insights into the prevalence of delirium, the optimization of interventions for managing delirium symptoms, their effectiveness and the impact of underlying factors on the reversibility of delirium in advanced cancer patients receiving palliative care. The review involved systematic searches of relevant databases including MEDLINE, CINAHL, ProQuest Nursing and Allied Health, and PsychInfo using refined search terms. Eight publications out of 614 studies originally searched were selected and critically reviewed. Their quality was assessed using Joanna Briggs Institute's Critical Appraisal Tool for Case Series. Data abstraction and content analysis were performed to synthesize the findings. Delirium is prevalent among advanced cancer patients in palliative care, with rates ranging from 10.3% to 24.1%. Pharmacotherapy and non-pharmacological interventions showed effectiveness in reducing delirium symptoms. Delirium was found to be reversible through palliative care interventions, antipsychotic medications, and exercise therapy. Effective delirium management is crucial in improving the quality of life of cancer patients. This review emphasizes the importance of subtype-specific treatments, standardized guidelines, and long-term follow-up studies. Implementing evidence-based individualized approaches to delirium management can optimize treatment efficacy and clinical outcomes in patients as well as improve the quality of care. Tailored interventions, standardized protocols, and further research are hereby recommended.

Calcium carbide (CaC2) is a commonly used ripening agent, although its primary use is in welding. Use of CaC2 in ripening fruits is considered harmful because it contains traces of phosphorous and arsenic. Aim of this study was to investigate associated risks and pathogenic (biochemical and histological) effects of consuming CaC2-ripened banana as a meal, before and/or during pregnancy on maternal and neonatal livers of adult female wistar rats. 36 healthy-looking, nulliparous, adult female wistar rats weighing between 150-200g were utilized. Rats were divided into 2 groups namely Group A and Group B. Each group had 4 subgroups (A1, A2, A3 and A4) for Group A as well as (B1, B2, B3 and B4) for Group B. Rats in group A received CaC2-ripened banana blended into their meal before and during pregnancy while group B rats received the same meal during pregnancy only. Subgroups A1 and B1 (positive control) received a naturally ripened banana with 0% CaC2, subgroups A2 and B2 received 1% CaC2-ripened bananas whereas subgroups A3 and B3 received 2% CaC2-ripened bananas. Subgroups A4 and B4 received CaC2-ripened bananas (bought from the market) randomly. The two groups A and B had one common negative control group which did not receive any banana (CaC2-ripened or non-CaC2-ripened) blended meal. Liver enzyme assay for group A showed that the positive control subgroup (A1) had mean AST (26.00±2.00 iu/L), ALT (33.00±3.00 iu/L), albumin (3.80±0.20 g/dL), direct (conjugated) bilirubin (1.10±0.10 mg/dL), and total bilirubin (1.95±0.15 mg/dL). These values were not significantly different (p≥0.05) compared with their respective values in the negative control subgroup except for ALT (p<0.05). Group B showed that subgroups B2, B3, and B4 had mean AST, ALT, albumin, direct bilirubin and total bilirubin values which were not significantly different (p≥0.05) compared with their respective values in the negative control subgroup. Histological findings confirm a damaging effect on the liver’s histoarchitecture of both mother and neonate. Consumption of CaC2-ripened banana before and/or during pregnancy has associated risks and pathogenic effects on maternal and neonatal livers.

Phoenix dactylifera (date fruit) is one of the nutritious and healthiest fruits due to its excellent nutritional properties, and many beneficial health and medicinal properties. The aim of this study was to evaluate sperm parameters and testicular histology of male Wistar rats treated with Phoenix dactylifera after consumption of a dose of local Mmahi salt (local salt of Ebonyi State origin, in Nigeria). Fresh date fruits, Mmahi salt, rat feed, clean drinking water, and 24 male wistar rats were the main materials for this study. The rats were divided into 6 groups: Group 1 (negative control group with 0% Mmahi salt (MS) and date fruit exposure), Group 2 (positive control I that received 75mg/kg of MS for 4 weeks), Group 3 (positive control II that received 75mg/kg of MS for 4 weeks followed by 2 weeks of self-recovery), Group 4 (that received 75mg/kg of MS for 4 weeks followed by 2 weeks of 200mg/kg of date fruit), Group 5 (that received 75mg/kg of MS for 4 weeks followed by 2 weeks of 400mg/kg of date fruit), and Group 6 (that received 75mg/kg of MS for 4 weeks followed by 2 weeks of 600mg/kg of date fruit). Actively motile spermatozoa in this study had mean±SEM in G1 (65.00±2.89), G2 (55.00±0.00), G3 (45.00±2.89), G4 (63.75±2.39), G5 (55.50±2.04), and G6 (58.75±2.39). Sluggishly motile spermatozoa were low in the groups treated with higher doses of date fruit extract, but least in G1. Non-motile spermatozoa were high in G2 and G3, but lower in G1, G5, and G6. G4, G5, and G6 had higher sperm counts compared to G2 and G3, although not significant. Histological examination showed in G1 - normal histology, in G2 - moderate spermatogenic arrest, in G3 - severe distortion of testicular architecture, in G4 - enhanced spermatogenesis, in G5 - moderate distortion of the seminiferous tubules, and G6 - enhanced spermatogenesis. A high dose of Mmahi salt is detrimental to sperm parameters and testicular histology. However, this study has shown that date fruit extract can improve sperm parameters and testicular histology in male wistar rats fed with Mmahi salt.
