Abscess and empyema caused by Legionella micdadei

Abstract: Legionella micdadei is the second most common species implicated in the occurrence of Legionella pneumonia (D. J. Bremer, Semin. Respir. Infect. 4:190-205, 1987). Although there has been a reported lung abscess caused by dual infection (L. micdadei and L. pneumophila), there are no known cases of L. micdadei as the only causative organism. We report a case of a patient with a lung abscess from which L. micdadei was the sole organism isolated.

“…Indeed, these bacteria are responsible for a significant percentage of both community-acquired and nosocomial pneumonias (23,40,63). Legionella infections are also manifest in other ways, including cases of pleurisy, endocarditis, pancreatitis, abscess, wound infection, colitis, peritonitis, pyelonephritis, and cellulitis (8,20,23,29,36,47). Finally, the flu-like illness known as Pontiac fever has been associated with increases in antibody titer to several Legionella species (22)(23)(24).…”

“…Legionella micdadei is the second most common cause of Legionnaires' disease and Pontiac fever (15,21,23). In addition, infection by this gram-negative organism can result in abscess formation and cellulitis (29,34). In contrast to Legionella pneumophila, the most common agent of legionellosis, L. micdadei infects only immunocompromised hosts (22,48).…”

“…Other Legionella species include, among others, L. bozemanii , and L. micdadei [2]. These other species, much rarer than L. pneumophila , can also exceptionally cause pneumonia with pleural empyema, which seems to be overrepresented compared to L. pneumophila [19,20]. Mycoplasma pneumonia , another intracellular bacteria known to cause pneumonia, has also been reported a few times as the causative agent of pleural empyema[21].…”
