Abiotrophia defectiva Infective Endocarditis: A Rare and Dangerous Cause of Endocarditis

Abstract: Infective endocarditis is an uncommon heart infection, typically involving heart valves. Abiotrophia defectiva is a rare cause of endocarditis, typically found within the GI tract, and is usually difficult to isolate and requires specialized media. We report a case of Abiotrophia defectiva endocarditis following a root canal.

“…In this case, the diagnosis of IE leading to severe aortic regurgitation was made by the utilization of POCUS. POCUS is a bedside imaging technique that provides real-time information about cardiac structures [3]. In this case, the presence of vegetation on the aortic valve was visualized using POCUS, which led to the early diagnosis of IE.…”

“…Blood cultures grew positive for bacilli (Abiotrophia defectiva). A. defectiva is a rare cause of IE which is mostly found in the oral cavity and the intestines and can be a cause of culture-negative endocarditis as it is very difficult to isolate and needs special media for culture [3]. We were advised by microbiology to treat him with Ceftriaxone and Gentamycin which resulted in significant improvement in his CRP.…”

“…IE attributed to A. defectiva typically exhibits a protracted course of progression. Despite the organism’s susceptibility to antimicrobial agents, a substantial proportion, nearly 50%, of cases necessitate surgical intervention for effective management [ 11 ]. In this specific case, multiple embolic events transpired in the brain and spleen.…”

“…1 Abiotrophia defectiva (A. defectiva) is a fastidious nutritionally variant microbe clinically implicated in osteomyelitis, brain abscess, and infective endocarditis. 2 This pathogen normally colonizes gastrointestinal tract and is a noted inhabitant of the oral flora. 2 A. defectiva is responsible for an estimated 17% mortality in cases complicated by infective endocarditis with multi system embolization.…”
