Aberrant internal carotid artery in the middle-ear space

Abstract: The incidence of an aberrant internal carotid artery in the middle ear is approximately one per cent and most patients are asymptomatic. We present two patients with an aberrant internal carotid artery who presented with pulsatile tinnitus and an intra-tympanic mass. Here we discuss the clinical presentation, relevant radiographic investigations and further management of these patients.

“…Entretanto, por seu potencial de complicações importantes, em geral, pode ser dispensada em favor dos outros métodos de imagem. Achados semelhantes podem ser demonstrados através da RNM, ajudando a caracterizar e definir a extensão da lesão, valendo-se da angiorressonância, com menor morbidade 1 . A necessidade de tratamento é controversa nos casos diagnosticados clínica ou radiologicamente; pode ser instituído para resolver os sintomas ou para prevenir complicações, como erosão ossicular ou sangramentos.…”

“…2,52 Iatrogenic injury to the petrous ICA during myringotomy and temporal bone surgeries has been reported. 1,5,7,8,12 Petrous ICA aneurysms of mycotic origin probably arise secondary to adventitial infection of the artery because of its close proximity to the middle ear and eustachian tube, rather than from hematogenous seeding. 44 Infectious processes, including chronic otomastoiditis, tonsillar and pharyngeal infections, and cholesteatomas have been implicated in the development of these aneurysms.…”

“…Aberrant ICA can mimic dehiscence of the jugular bulb, glomus tumours, haemangiomas, and cholesterol granulomas [2][3][4] . In the past, it was often found during ear surgery or biopsy and may cause massive bleeding and life-threatening complications [5] . Because the symptoms and signs are often nonspecific or absent, clinical diagnosis of aberrant ICA is very difficult.…”
