PREFACEIn Australia, everyone shares a fundamental right to safe and high-quality health care. This is defined in the Australian Charter of Healthcare Rights, 1 which all healthcare systems must strive to uphold. The Australian Charter of Healthcare Rights summarises the basic rights of patients and consumers when accessing healthcare services, including access, safety, respect, partnership, information, privacy, and the ability to give feedback. The provision of pharmacy services must encompass the Charter to deliver effective, efficient, timely, and equitable person-centred care and pain management.This Standard references, and relies upon, SHPA Standards of Practice for Clinical Pharmacy Services 2 as the foremost Standard. This Standard supersedes the previous 'Standard of practice in pain management for pharmacy services', 3 and because pain management exists in almost all practice specialties, this Standard may overlap with others. Depending on the area of specialty practice, it may be advisable to refer to additional Standards of Practice.The use of the word 'specialisation' in this Standard is in line with the National Competency Standards Framework for Pharmacists in Australia, 4 where 'specialisation' refers to the scope of practice rather than the level of performance. 'Specialisation' in and of itself does not confer additional expertise.

SUMMARY Medicines stewardship refers to coordinated strategies and interventions to optimise medicines use, usually within a specific therapeutic area. Medicines stewardship programs can reduce variations in practice and improve patient outcomes. Therapeutic domains for medicines stewardship are chosen to address frequently used drug classes associated with a high risk of adverse outcomes. Some examples include antimicrobial, opioid analgesic, anticoagulation and psychotropic stewardship. Common elements of successful stewardship programs include multidisciplinary leadership, stakeholder engagement, tailored communication strategies, behavioural changes, implementation science methodologies, and ongoing program monitoring, evaluation and reporting. Medicines stewardship is a continual quality improvement process that requires ongoing support and resources, as well as clinician and consumer engagement, to remain sustainable. It is critical for hospital-based medicines stewardship programs to consider impacts on care in the community when making and communicating changes to patient therapy. This ensures that stewardship efforts are sustained across transitions of care.

Medication‐related harm can occur during transitions of care. Revised Home Medicines Review (HMR) and Residential Medication Management Review (RMMR) program rules were published in April 2020 which allowed provision for some hospital medical practitioners to refer at‐risk patients for medication review. In turn, the Society of Hospital Pharmacists of Australia's (SHPA's) Transitions of Care and Primary Care Leadership Committee developed a framework to support hospitals facilitating Hospital‐Initiated Medication Reviews (HIMRs) via three pathways: HMR, RMMR, and Hospital Outreach Medication Review. Following the compilation of draft barriers and enablers to implementation of the SHPA HIMR framework, refinement occurred after broad consultation with hospital‐ and primary care‐based pharmacists with transitions of care experience. The finalised list of barriers and enablers can inform broadscale implementation of the SHPA HIMR framework to reduce medication‐related harm when high‐risk patients transition from hospital to primary care and aged care.
