Purpose This is a retrospective analysis of the impact of moderate dysplasia at the resection margin for early stage cancer of the oral tongue. Materials and Methods Patients with T1-2N0 oral tongue cancer treated with surgery alone at Fox Chase Cancer Center (FCCC) from 1990 – 2010 were reviewed. Tumor and margin characteristics were abstracted from the pathology report. Overall survival (OS), disease-free survival (DFS) and local control (LC) were calculated using the Kaplan Meier method. Predictors of LC, OS and DFS were analyzed. Results 126 patients met the inclusion criteria. Dysplasia was present at the final margin in 36% of the cases (severe: 9%, moderate: 15%, mild: 12%). Median follow-up was 52 months. 3 and 5-year actuarial LC for the entire cohort was 77 and 73%, respectively. Actuarial 5-year LC and DFS were significantly worse for patients with moderate or severe dysplasia at the margin vs. none or mild dysplasia at the margin (49 v 82%, p = 0.005 and 49 v 80%, p = 0.008, respectively); 3-year comparisons were not significant. When analyzed separately, the detrimental local effect of moderate dysplasia at the margin persisted (p = 0.02) and the effect of severe dysplasia at the margin was approaching significance (p = 0.1). Mild dysplasia at the margin did not significantly impair LC or DFS. Multivariate analysis demonstrated worse LC (HR: 2.99, p=0.006) and DFS (HR: 2.84, p=0.008) associated with severe or moderate dysplasia at the margin. Conclusions Both severe and moderate dysplasia at the margin appear to be correlated with inferior LC and DFS. Additional therapy may be justified, despite added morbidity.

A retrospective analysis was undertaken of 65 patients with long-term follow-up for laryngeal squamous dysplasia. Based on the degree of dysplasia demonstrated on initial biopsy, 0 of 6 patients showing hyperkeratosis without dysplasia, 3 of 26 patients (12%) showing mild dysplasia, 5 of 15 patients (33%) showing moderate dysplasia, 4 of 9 patients (44%) showing severe dysplasia, and 1 of 9 patients (11%) showing carcinoma in situ eventually progressed to invasive carcinoma. An analysis was made of the impact of various treatment modalities in 33 patients demonstrating moderate dysplasia, severe dysplasia, or carcinoma in situ. Invasive carcinoma developed in 10 of 21 patients (48%) treated endoscopically and 0 of 12 patients treated by more aggressive therapy, including external beam radiotherapy, partial laryngectomy, or total laryngectomy. Of the patients in the endoscopic therapy group who developed invasive carcinoma, all were salvaged successfully. The overall rate of laryngeal preservation was 15 of 21 patients (71%) in the endoscopic treatment group and 11 of 12 patients (92%) in the aggressive treatment group. This difference is not statistically significant. We conclude that there is a moderately high rate of progression to invasive carcinoma in patients undergoing repeated endoscopic therapy for intraepithelial neoplasms of the larynx. However, with close, long-term follow-up, patients undergoing endoscopic therapy have an overall outcome similar to that in patients treated with partial laryngectomy or radiotherapy prior to developing invasive disease.

Die Ergebnisse der Behandlungen von 72 Patienten mit histopathologisch bestÄtigtem Carcinoma in situ (CIS) des Larynx in den Jahren 1978–1988 wurden retrospektiv untersucht. Die Untersuchung umfasste 8,1% aller Patienten mit Plattenepithelkarzinom (SCC) des Larynx. Entsprechend dem Behandlungsprotokoll der Klinik wurde innerhalb von 2–3 Monaten eine zweite Biopsie entnommen, worauf die Planung der Therapie erfolgte. Bei 14 Patienten (19%) zeigte sich in der Zweitbiopsie eine Progression zu einem invasiven SCC. Ein Patient starb vor der zweiten Biopsie an einem Prostata-karzinom. Die Behandlungsergebnisse der verbliebenen 57 Patienten mit CIS wurden retrospektiv untersucht. 29 Patienten erhielten Radiotherapie; davon entwickelten 6 (21 %) ein Rezidiv, und zwar in alien Fallen ein SCC. 13 Patienten wurden endoskopisch-chirurgisch behandelt; davon entwickelten 8 (60%) ein Rezidiv, in 5 Fallen CIS und in 3 Fallen SCC. Bei 15 Patienten verhielt man sich expektativ, da die zweite Biopsie nur eine Dysplasie zeigte, jedoch wurde auch bei diesen Patienten in 7 (47%) Fallen spÄter ein Rezidiv gefunden, in 5 Fallen CIS und in 2 SCC. 10 der 21 Rezidive entwickelten sich innerhalb von 2 Jahren nach Behandlungsbeginn. Alle Patienten, welche ein invasives SCC entwickelten, erhielten Radiotherapie. Die Zeit bis zum Auftreten des Rezidivs reichte von 9 Monaten bis zu 11 Jahren, durchschnittlich 30 Monate. Die durchschnittliche Beobachtungszeit betrug 6 Jahre und 5 Monate. In der chirurgisch behandelten Gruppe waren 6 (46%) Patienten durch das alleinige endoskopisch-chirurgische Vorgehen geheilt. In der expektativ beobachteten Gruppe waren 10 (66%) Patienten – bioptisch gesichert – rezidivfrei geblieben. Diese Studie zeigt, dass Radiotherapie eine effiziente Behandlungsmethode fÜr das CIS des Larynx darstellt, dass jedoch auch eine konservative Einstellung der Behandlung bei engmaschigen Kontrollen der Patienten ihre Berechtigung hat.
