A validation of the new definition of drug‐resistant epilepsy by the International League Against Epilepsy

Abstract: SUMMARYObjective: To establish applicability, the recently proposed International League Against Epilepsy (ILAE) consensus on drug-resistant epilepsy (DRE) requires testing in clinical and research settings. This study evaluates the reliability and validity of these criteria in a clinical population. Methods: In phase I, two independent evaluators reviewed 97 randomly selected medical records of patients with epilepsy at two separate intervals. Both ILEA consensus and standard diagnostic criteria were employed… Show more

“…7 Intellectual disability was defined as a global intelligent quotient (IQ) < 70 on standard psychological testing unless the patient was too impaired for testing. Intractable epilepsy was defined as failure of three AEDs and at least one seizure each 3 months in the last year of follow-up.…”

“…Drug resistant (or medically refractory) epilepsy is defined as “failure of adequate trials of two tolerated and appropriately chosen and used AED schedules (whether as monotherapies or in combination) to achieve sustained seizure freedom” by the International League Against Epilepsy (ILAE) in their 2009 guideline . Approximately 30% of adults newly diagnosed with epilepsy will become drug resistant . In this population, the chances of becoming seizure‐free by trying additional drugs are quite low.…”

“…The earlier AED treatment is started; however, the better the potential outcome may be for seizure control 21, 22, 23. Recurrent epileptic seizures can increase epileptogenesis and drug resistance in a subgroup of patients 24, 25. Prolonged and acute repetitive seizures can increase patient morbidity and require prolonged hospitalization with associated financial burden.…”
