A Twenty-year Retrospective Study of Pediatric Tuberculosis in Two Tertiary Hospitals in Rome

Abstract: Our study emphasizes the difficulty in managing children with suspected TB, because the absence of constitutional symptoms cannot exclude TB, and bacteriologic confirmation is the exception. Immunologic diagnosis can be a valuable tool to identify TB-infected children because the quantiferon test showed high sensitivity in all age groups. This is of primary importance because early identification of children with latent tuberculous infection and appropriate chemoprophylaxis represent, to date, the most importa… Show more

“…In our study the majority of the TB cases (78.6%) were associated with pulmonary involvement supporting the findings of the recent papers from Italy, Denmark and Turkey 11,14,17,18. Nearly 2/3 of the patients had EPTB alone suggesting that EPTB could be underdiagnosed in the absence of pulmonary findings.…”

“…Data including demographic characteristics, clinical history, microbiology, imaging studies, medications and outcomes of the patients were collected from medical records. The microbiologically confirmed active TB was defined as the presence of at least one positive clinical specimen for Mycobacterium tuberculosis (Mtb) in culture, or positive AFB smear microscopy, or histological confirmation of caseating granulomas 14. Probable tuberculosis was defined as the presence of suggestive radiologic signs in the absence of bacteriologic confirmation, or radiologic certainty, positive nucleic acid amplification test for Mtb and good clinical response to antitubercular treatment 15…”

“…Athelectasia usually seen in middle lobe presented as radiopaque triangular lesions producing lung volume reduction and mediastinal distorsion are frequently seen after mediastinal lymph node enlargement, or pleural effusion, are also common findings especially among patients with an evolved progressive primary infection [20,21]. The absence of pathognomonic radiological findings, and the difficulty to get a positive Mycobacterial culture [22,23] makes it hard to get a definitive diagnosis in children with PTB. Miliary TB, a systemic form of the disease that typically affects children 6 to 11 years old, characterizes by a micronodular pattern consisting of well-defined nodules-in the shape of seeds of millet-uniformly distributed in all lobes, and it is seen usually 6 weeks after primary infection.…”
