A thirty-year survey of the reconstructive surgical treatment of aortoiliac occlusive disease

Abstract: With the view of assessing functional durability and the factors that influence or determine it, we reviewed the clinical course of 1748 reconstructive operations performed between Jan. 1, 1954, and Dec. 31, 1983 in the treatment of 1647 patients with aortoiliac occlusive disease (AIOD). Disabling intermittent claudication (in 65.6%), ischemic rest pain and/ or pregangrene (in 20.7%), and ischemic gangrene (in 13.7%) were the operative indications. Patency proven by angiography was the criterion of success. Fo… Show more

“…It has been proven in literature that hypotension alone can be an external cause for acute thrombosis of an arterial graft [23,24]. In our case, our mean arterial pressure remain between 60-87 mmHg which is in normal limits for THA [25].…”

“…[3][4][5][6][7][8][9][10] However, cross-clamping, which is required to control blood flow in all these treatment options, might be difficult in some cases. Cross-clamping the calcified aorta might lead to plaque rupture and embolization of plaque material or to aortic wall injury that might be impossible to repair, especially in heavily calcified aortas.…”

“…The durability of these procedures is excellent (85% to 90% at 5 years). 1 Reported operative mortality is 3.3%, whereas perioperative morbidity is 8.3%. [1][2][3] In spite of excellent patency rates, surgery is associated with anesthesia, prolonged hospitalization and recovery time, and loss of sexual function.…”

[1][2][3] In spite of excellent patency rates, surgery is associated with anesthesia, prolonged hospitalization and recovery time, and loss of sexual function. Kissing balloon angioplasty is an alternative to surgery 1,2 , but there is significant incidence of dissection, thrombosis and residual stenosis 4,5 .…”
