A Tale of Two Gonorrhea Epidemics: Results from the STD Surveillance Network

Abstract: These data identified two concurrent gonorrhea epidemics in minority populations: a young, black, heterosexual epidemic with frequently reported recent incarceration, and an older, mostly white MSM epidemic with more frequently reported anonymous sex, Internet use to meet sex partners, and methamphetamine use.

“…Although several preventive strategies exist, their effectiveness remains suboptimal for multiple reasons, as has been demonstrated in many places by the persistence of sexually transmitted infections [20,34,35]. In a recent study completed in Buenos Aires, 11% of PLWHA presented with a new sexually transmitted infection after being diagnosed with HIV [36].…”

“…We used a Bayesian approach to calibrate the model to the estimated prevalence of gonorrhea, the rate of gonorrhea cases, and the proportion of cases presenting symptoms among MSM in the US. We examined alternative strategies to inform the timing of shifts in first-line treatment [20]). In these simulated trajectories, the first-line treatment is changed when more than 5% of the annual gonorrhea cases are resistant to the first-line drug.…”
